About iStent Trabecular Bypass
The iStent is a tiny titanium implant placed in the eye's natural drainage channel to lower pressure. It is for mild to moderate open-angle glaucoma, usually combined with cataract surgery.
iStent implantation is a day case under local or topical anaesthesia. Through the same small corneal incision used for cataract surgery, the surgeon inserts a microscopic stent into Schlemm's canal using a viewing lens. The stent bypasses the blocked part of the drainage system so fluid can leave the eye more easily.
Usually a good fit if
- Mild to moderate open-angle glaucoma undergoing cataract surgery.
- Wanting fewer glaucoma drops.
- Open angles on gonioscopy.
- Pressure target that does not require a very low level.
May need to wait if
- Angle-closure or neovascular glaucoma.
- Advanced glaucoma needing very low pressure.
- Active inflammation or infection.
- Expectation of a cure without drops.
How to prepare
- Bring visual field tests, optic nerve scans and your pressure history.
- Review glaucoma drops with your doctor; do not stop them yourself.
- Undergo cataract measurements if combined surgery is planned.
- Ask whether your angle has been checked by gonioscopy.
- Arrange transport and early reviews.
Questions to ask your surgeon
- Is an iStent enough for my glaucoma stage?
- Will you place one or more stents?
- What pressure reduction and drop reduction can I expect?
- What if the pressure stays high?
- Does this prevent later trabeculectomy?
- Who will monitor my pressure at home?
Red flags when comparing hospitals
- No gonioscopy or field test before advising.
- Promising freedom from drops.
- No plan for follow-up pressure checks.
- Stent model not documented.
