About Hemorrhoidectomy (Surgical)
Surgical haemorrhoidectomy removes large or prolapsing haemorrhoids (piles) that do not respond to office treatments. It is used for grade III and IV piles and for acute thrombosed piles.
The operation takes about twenty to forty minutes under spinal or general anaesthesia. With a retractor, the surgeon finds each pile cushion, clamps its base and cuts away the haemorrhoidal tissue while keeping bridges of healthy skin and lining between the wounds. Wounds are either closed with stitches or left open, depending on the technique.
Usually a good fit if
- Grade III or IV haemorrhoids that prolapse and need manual replacement.
- Piles that keep bleeding or cause problems despite banding or injections.
- Mixed internal and external haemorrhoids.
- Acutely thrombosed piles in selected cases.
May need to wait if
- Active anal infection or Crohn disease that needs treatment first.
- Weak sphincter or incontinence, since surgery might worsen it.
- Uncorrected clotting problems or portal hypertension with rectal varices.
- Piles that are small enough to respond to banding.
How to prepare
- Your doctor will examine you with a proctoscope and may recommend colonoscopy if you are older or have bleeding that is unexplained.
- Start fibre and plenty of fluids before surgery to keep stools soft.
- Review blood thinners and other medicines with your doctor; do not stop them yourself.
- Bowel preparation or an enema may be advised.
- Pack loose underwear, pads, stool softener and painkillers.
Questions to ask your surgeon
- Is surgery needed or can banding or other treatments work?
- Will you use excision or a stapled technique?
- What level of pain should I expect and how will it be controlled?
- What are the risks of bleeding, narrowing or incontinence?
- Do I need a colonoscopy first?
- What follow-up is needed at home?
Red flags when comparing hospitals
- No examination or explanation of the grade of piles.
- Surgery offered with no trial of simpler treatments.
- No discussion of pain control.
- Promises of painless surgery.
- No plan for bowel regulation afterwards.
