About Elbow Replacement

Elbow replacement swaps a damaged elbow joint for a metal and plastic implant. It is for people with severe arthritis, usually rheumatoid, or a badly broken elbow that cannot be repaired.

Through an incision at the back of the elbow, the surgeon removes the damaged joint surfaces and fits a hinged implant into the upper arm and forearm bones. It is done under regional or general anaesthesia. Because the joint is complex and replacements are less common, results depend heavily on a surgeon who does this regularly.

Usually a good fit if

  • Severe rheumatoid or other arthritis of the elbow with pain and loss of movement.
  • Complex fractures of the elbow in older adults.
  • Failed previous elbow surgery with joint damage.
  • Low-demand use of the arm, mainly for daily activities.

May need to wait if

  • Active infection in or around the elbow.
  • Heavy manual work or sports requiring lifting, which can loosen the implant.
  • Poor skin or nerve problems around the elbow.
  • Young age with high expected demand.
How to prepare
  1. Have X-rays of the elbow and sometimes CT to plan bone stock and implant size.
  2. Mention inflammatory arthritis, steroid use, diabetes and previous elbow surgery or infection.
  3. Review blood thinners, immunosuppressants and arthritis medicines with your doctor; do not change them yourself.
  4. Practise one-handed daily tasks, as you cannot lift with the arm afterwards.
  5. Arrange help at home and check fitness to fly.
Questions to ask your surgeon
  1. How many elbow replacements do you perform each year?
  2. Which implant do you use, linked or unlinked, and why?
  3. What lifting restrictions will I have for life?
  4. What are the risks of loosening, infection, nerve injury and revision?
  5. How long will the implant last?
  6. What therapy can I do at home?
Red flags when comparing hospitals
  • Surgeon with few elbow replacements; it is uncommon and volume matters.
  • No discussion of lifting limits.
  • No protection of the ulnar nerve or plan for its care.
  • No follow-up plan for X-rays.
