About Bentall Procedure (Aortic Root Replacement)
The Bentall procedure replaces the aortic valve, the aortic root and part of the ascending aorta in one operation. It is used when both the valve and the aorta beside it are diseased or widened.
Under general anaesthesia and with a heart-lung machine, the surgeon opens the chest and removes the diseased valve and root. A composite graft, a fabric tube with a valve sewn into it, is stitched in place, and the coronary arteries are reattached to it. The valve can be mechanical or tissue, which is chosen according to age, lifestyle and ability to take blood thinners.
Usually a good fit if
- Aortic root aneurysm, usually around 5.0 to 5.5 centimetres or more, with a diseased or leaking aortic valve.
- Conditions such as Marfan syndrome, bicuspid valve disease or root infection needing root replacement.
- A valve unsuitable for repair, so a valved conduit is needed.
- Fitness for open-heart surgery.
May need to wait if
- Root disease where a valve-sparing operation is possible and preferred, such as in a young patient with a normal valve.
- Severe heart, lung or kidney failure making surgery too high-risk.
- Active infection other than the heart condition, until treated.
- Inability to take warfarin reliably if a mechanical valve is planned.
How to prepare
- Bring CT or MR angiography and echocardiography reports showing the root and valve.
- Have coronary angiography or CT, blood tests, lung function and kidney checks as your team requests.
- Review blood thinners, blood-pressure drugs and diabetes medicines with your doctor, who will tell you what to adjust.
- Decide with your surgeon between a mechanical and a tissue valve based on age, pregnancy plans and ability to take warfarin.
- Plan a long stay near the hospital, including help at home and rehabilitation.
Questions to ask your surgeon
- Is a Bentall right for me or could my valve be spared with a David procedure?
- Mechanical or tissue valve, and what are the lifelong implications for me?
- How many Bentall operations does your team perform each year?
- How will my coronary arteries be reattached, and what is your experience?
- What anticoagulation monitoring will I need at home?
- What scan schedule do I follow for the rest of the aorta?
Red flags when comparing hospitals
- No discussion of valve-sparing options for suitable patients.
- The surgeon does not explain mechanical versus tissue valve trade-offs.
- No cardiac intensive care.
- No plan for warfarin monitoring after discharge if a mechanical valve is used.
- Surgeon unable to quote their own root-surgery experience.

Is Bentall Procedure (Aortic Root Replacement) cheaper in Malaysia than in other countries?

How long do I need to stay in Malaysia?
Most patients spend about 9 nights in hospital. Full recovery takes about 56 days; your surgeon will tell you when you are fit to fly home.
How long does a medical visa for Malaysia take?
Visa-on-arrival (most nationalities) + Medical Visa for extended stays: processing takes about 3 days and the visa is valid for 30 days. We help with the hospital invitation letter.
Mechanical or tissue valve?
Mechanical valves last very long but need lifelong warfarin. Tissue valves need less or no warfarin but may wear out, sometimes needing replacement. Your surgeon helps you choose.
Why are the coronary arteries moved?
They arise from the aortic root, so they must be reattached to the new graft to keep the heart supplied with blood.
Is the Bentall procedure a major operation?
Yes, it is complex open-heart surgery, usually requiring about a week to ten days in hospital and months to fully recover.
Will I need follow-up scans?
Yes. Regular echo and aortic imaging check the graft, valve and the rest of the aorta.
