About ICSI (Intracytoplasmic Sperm Injection)
ICSI is a laboratory fertilisation technique used within IVF, where one sperm is placed directly into an egg. It is mainly recommended when semen quality is poor or earlier fertilisation has failed.
ICSI is added to an ordinary IVF cycle, so you first have hormone injections to grow several eggs, followed by a short egg collection under sedation. An embryologist then selects one healthy-looking sperm for each mature egg and injects it with a fine glass needle. Embryos are cultured for several days before one is transferred to the womb or frozen.
Usually a good fit if
- Significant male factor infertility such as low count, poor motility or abnormal sperm shape.
- Surgically retrieved sperm (for example TESE or MESA) or frozen sperm of limited quantity.
- Previous IVF cycle with failed or very low fertilisation.
- Few eggs available, where every egg must have the best chance of fertilising.
May need to wait if
- Normal semen analysis with no earlier fertilisation problem, where standard IVF may work equally well.
- Uncontrolled medical conditions in the woman that make pregnancy or sedation unsafe until optimised.
- Untreated uterine problems, such as a large polyp or fibroid distorting the cavity, that need correction first.
- Unrealistic expectations of guaranteed pregnancy, since ICSI helps fertilisation but not embryo quality or implantation.
How to prepare
- Both partners usually need recent hormone tests, an infection screen, and a semen analysis; your clinic may add a genetic or karyotype test for severe male factor.
- Review all medicines and supplements with your doctor before starting stimulation, and do not change any on your own.
- Plan to stay near the clinic for roughly two to three weeks for scans, trigger injection, egg collection and transfer.
- Bring previous semen reports, scan reports and earlier IVF records, with certified translations if they are not in English.
- Fast from midnight before egg collection if sedation is planned, and arrange for someone to accompany you afterwards.
Questions to ask your surgeon
- Is ICSI actually needed for us, or would conventional IVF fertilisation be reasonable?
- What is your fertilisation rate with ICSI, and how many embryos usually reach blastocyst stage in patients like me?
- How many embryos will you transfer, and what is your policy on freezing the rest?
- Do you offer sperm DNA fragmentation testing or sperm selection techniques before ICSI?
- What is the plan if no eggs fertilise, and who will review the cycle with us afterwards?
- What follow-up monitoring and pregnancy testing can be done at home after transfer?
Red flags when comparing hospitals
- The clinic recommends ICSI for every couple without explaining the male-factor reason.
- No embryologist details or laboratory accreditation can be shown when you ask.
- Pressure to transfer many embryos at once to improve the headline pregnancy rate.
- No written record of fertilisation and embryo development after the cycle.
- Unclear explanation of ovarian hyperstimulation risk and how it is monitored.
Questions about ICSI (Intracytoplasmic Sperm Injection) in India
How much does ICSI (Intracytoplasmic Sperm Injection) cost in India in 2026?
Between $4,250 and $9,200 at most hospitals, with a typical price of $6,750. This is based on 8 hospital prices in India in our database. Your final price depends on the hospital, room type and implant; we send written quotes that list what is included.
Is ICSI (Intracytoplasmic Sperm Injection) cheaper in India than in other countries?
India ranks 1 of 9 destinations we compare for this procedure. The lowest typical price is in India at about $6,740.
How long does a medical visa for India take?
Medical Visa (e-Medical available): processing takes about 4 days and the visa is valid for 60 days. We help with the hospital invitation letter.
Does ICSI harm the baby?
ICSI babies are born healthy in the large majority of cases. Some studies suggest a small increase in certain birth defects and a risk that male children inherit male-factor infertility. Your doctor can discuss genetic testing if the cause is severe.
Does ICSI guarantee fertilisation?
No. Most mature eggs fertilise, but a small number do not, and occasionally none do. Egg quality also affects results, which is why embryo development is monitored daily.
Can ICSI be used with frozen sperm?
Yes. Even a few motile sperm, including ones retrieved surgically or thawed from storage, can be used because only one is needed per egg.
Is the egg collection painful?
It is done under sedation, so you should feel little. Some cramping and bloating for a day or two is common afterwards.
