About ICD (Implantable Cardioverter Defibrillator)
An implantable cardioverter defibrillator is a small device that watches your heart rhythm and delivers a shock or pacing to stop dangerous fast rhythms. It is for people at risk of sudden cardiac arrest.
Under local anaesthesia with sedation, the doctor makes a small cut below the collarbone, guides one or two leads through a vein into the heart and connects them to a generator placed under the skin. The device is tested before you wake. A subcutaneous version can be placed without leads in the heart, and suits some younger people. The procedure takes about one to two hours.
Usually a good fit if
- Survivors of cardiac arrest or dangerous ventricular rhythms (secondary prevention).
- Weak heart pumping, often ejection fraction 35 percent or lower, at risk of dangerous rhythms despite medicines (primary prevention).
- Inherited heart rhythm conditions with high risk.
- A reasonable life expectancy beyond a year.
May need to wait if
- Active infection, which must be treated first.
- Reversible causes of the rhythm problem not yet treated.
- Very advanced illness with limited life expectancy.
- Inability to attend device checks.
How to prepare
- Have an ECG, echocardiogram, blood tests and chest X-ray; bring documentation of ejection fraction or arrhythmia history.
- Review blood thinners, heart and diabetes medicines with your doctor, who will tell you what to adjust.
- Report any infection, fever or skin problem near the chest.
- Fast for the hours the team specifies.
- Plan for a device identification card and remote monitoring set-up.
Questions to ask your surgeon
- Do I need an ICD alone, or CRT-D or a subcutaneous ICD?
- Which device and lead type will you use and why?
- What is my chance of receiving a shock?
- How do you reduce the risk of infection and lead problems?
- Can my home clinic follow this brand?
- What are my driving and work restrictions?
Red flags when comparing hospitals
- ICD advised without echo or rhythm evidence supporting need.
- No device clinic or remote monitoring arranged.
- Brand chosen without explanation.
- Infection prevention protocol not described.
- Inadequate counselling about shocks.
