Warfarin
A long-established anticoagulant requiring regular INR blood-test monitoring.
Warfarin (Coumadin) blocks vitamin K-dependent clotting factor production. Dosing is individualized via INR (international normalized ratio) blood testing.
What this means for you: warfarin is paused 4-5 days before most surgery, with bridging by short-acting heparin where stroke risk is high. Confirm your bridging plan in writing with the prescribing physician.
