Hepatitis B / C Cirrhosis

Cirrhosis from hepatitis B or C is scarring of the liver caused by long-term viral infection. It develops over decades and affects people who were infected through birth, blood or other exposure, often without knowing.

Sobre hepatitis b / c cirrhosis

O que você precisa saber

Early cirrhosis may cause no symptoms; later signs include tiredness, jaundice, abdominal swelling, bruising and confusion. Diagnosis combines blood tests for the virus, scans, elastography and sometimes biopsy. Modern antiviral drugs can halt further damage, but advanced cirrhosis may progress to liver failure or liver cancer.

Antivirals cure hepatitis C in most people and suppress hepatitis B, and care includes managing fluid, bleeding risk and screening for liver cancer. When the liver fails, transplantation is the definitive treatment, and living donor liver transplant offers an alternative where waiting lists are long. Patients often travel abroad for transplant.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Viral tests: hepatitis B surface antigen and DNA level, or hepatitis C RNA and genotype, with all previous antiviral treatment records.
  • Liver function tests, INR, albumin, bilirubin, platelet count and calculated Child-Pugh and MELD scores.
  • Liver ultrasound, CT or MRI reports, since these screen for liver cancer and show portal hypertension.
  • An endoscopy report for oesophageal varices and a record of any ascites, bleeding or confusion episodes.
  • Alpha-fetoprotein results, kidney function and a list of current medicines.

Segunda opinião

Quando buscar uma segunda avaliação

  • You have been told you need a transplant but have not tried or completed modern antiviral treatment.
  • A liver lesion has been found and it is unclear whether it is cancer.
  • You have been told you are too sick or too old for transplant, and a transplant centre has not reviewed you.
  • Possible donors have been identified and you want an independent view on suitability and risk.

Perguntas para fazer

  1. How advanced is my cirrhosis, and is it compensated or decompensated?
  2. Will antiviral treatment stop the virus, and can liver function improve on it?
  3. Have you checked for liver cancer and varices?
  4. Do I meet criteria for a liver transplant, and how urgent is it?
  5. What are the risks to the donor and the recipient in living donor transplant?
  6. How will the virus be controlled after transplant to prevent it from returning?

Perspectiva

Prognóstico realista

Outlook depends on whether the liver still functions adequately. In compensated cirrhosis, effective antiviral treatment can halt progression and may improve liver health, though the risk of liver cancer remains and needs regular scanning. Once complications such as fluid, bleeding or confusion appear, prognosis worsens and transplant becomes the main option. Transplant for hepatitis-related cirrhosis has good survival rates when the virus is controlled afterwards. Alcohol and other liver conditions influence the result.

Can antiviral treatment cure cirrhosis?

It cannot reverse advanced scarring, but it can stop further damage, and hepatitis C is usually cured. Liver health may improve.

Do I still need cancer screening after treatment?

Yes. People with cirrhosis remain at risk of liver cancer even after the virus is controlled, so regular ultrasound is advised.

What is living donor liver transplant?

A part of a healthy donor's liver is transplanted. Both livers regrow, but the donor operation carries risk.

Will hepatitis return after transplant?

Hepatitis C is usually cured before or after transplant. Hepatitis B is controlled with antivirals to prevent recurrence.

Clinical context

Working up hepatitis b / c cirrhosis properly

A condition like hepatitis b / c cirrhosis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a transplante de órgãos specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or transplante de órgãos specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
