Sobre insuficiência hepática

O que você precisa saber

Signs include jaundice, abdominal swelling, confusion or drowsiness, easy bleeding, vomiting blood and marked tiredness. Diagnosis uses blood tests for liver enzymes, bilirubin and clotting, imaging, and a search for the cause such as viruses, drugs, alcohol or autoimmune disease. Acute failure can worsen within days, while chronic failure develops over years with periods of decompensation.

Treatment focuses on the cause and complications: stopping alcohol, antivirals, medicines for fluid and confusion, and supportive intensive care in acute cases. When liver function cannot recover, liver transplantation is the definitive treatment, using a deceased or living donor. Candidates are assessed for suitability and listed according to urgency. Treatment abroad may be considered for living donor transplant at an experienced centre, where law allows.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Cause of liver disease, such as hepatitis B or C, alcohol, autoimmune disease, fatty liver or a drug reaction, with relevant test results.
  • Recent blood tests: bilirubin, INR, albumin, creatinine, sodium and platelet count, used to calculate the MELD score.
  • Ultrasound or CT of the liver and portal vein, to exclude liver tumour and blood clots.
  • Hospital summaries of complications such as bleeding varices, fluid in the abdomen (ascites), confusion or infections.
  • Blood group, infection screening, heart and lung assessment, and details of any potential living donor.

Segunda opinião

Quando buscar uma segunda avaliação

  • You have been told you are too sick or too well for transplant but no transplant centre has formally assessed you.
  • Your liver failure came on suddenly in a previously healthy person, where urgent transplant assessment is essential.
  • A relative wishes to donate part of their liver and you want to review risks to the donor.
  • You have alcohol-related disease and want to understand the criteria and support required.

Perguntas para fazer

  1. What is the cause of my liver failure, and is it treatable or reversible?
  2. What is my MELD score and what does it mean for timing of transplant?
  3. Am I suitable for transplant, and are there medical or social issues that must be addressed first?
  4. Is a living-donor transplant possible, and what are the risks for the donor?
  5. What is the plan for the complications I have, such as fluid or confusion?
  6. What lifelong medicines and monitoring will be needed after a transplant?

Perspectiva

Prognóstico realista

Liver failure is life-threatening, and outcomes depend on the cause, how advanced it is and whether complications such as infection or kidney failure are present. Transplant offers the only cure for end-stage disease, and after a successful operation most recipients survive many years with good quality of life. Some acute cases recover if the cause is removed in time. Recipients need lifelong anti-rejection medicines and regular checks.

Is liver failure always permanent?

No. Acute liver failure can recover if the cause is treated or the liver regenerates. Chronic end-stage failure from cirrhosis is generally not reversible.

Can a family member donate part of their liver?

Yes, living-donor liver transplant is possible with a compatible, healthy donor. The donor's liver regrows, but donation carries real surgical risks that are carefully assessed.

Can I travel while waiting for a transplant?

Only with your hepatologist's agreement. Travel is risky if you have a high MELD score, fluid, confusion or bleeding, so plan transport and care carefully.

Clinical context

Working up insuficiência hepática properly

A condition like insuficiência hepática is rarely well-served by jumping straight to treatment shopping. The sequence below is what a transplante de órgãos specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or transplante de órgãos specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
