About Subtotal Gastrectomy
Subtotal gastrectomy removes the lower part of the stomach, usually for stomach cancer in that area, and reconnects the remaining stomach to the small bowel. Nearby lymph nodes are removed as well.
The surgeon takes the lower two thirds or so of the stomach, along with the lymph nodes around it, and joins what remains to the small bowel, often in a Billroth II or Roux-en-Y reconstruction. It can be performed open or laparoscopically. Chemotherapy is often given before and after surgery for more advanced cancers.
Usually a good fit if
- Stomach cancer in the lower part of the stomach that can be removed with clear margins while leaving a stomach remnant.
- Fit enough for abdominal surgery, with nutrition addressed.
- A multidisciplinary team recommends surgery, with or without chemotherapy before and after.
May need to wait if
- Cancers in the upper stomach usually need total gastrectomy instead.
- Spread to the liver, lungs or abdominal lining often calls for drug therapy first.
- Severe malnutrition, heart or lung disease needs optimisation.
How to prepare
- Bring endoscopy, biopsy, CT and staging laparoscopy findings, plus results of any chemotherapy given beforehand.
- Have blood tests for anaemia and nutrition; correct low iron or protein levels before surgery.
- Ask your doctor about adjusting blood thinners, diabetes medicines and stomach acid treatment.
- Meet the dietitian to plan small frequent meals and vitamin supplementation.
- Stop smoking and alcohol, and arrange help at home for the first weeks.
Questions to ask your surgeon
- Is subtotal or total gastrectomy right for my tumour position, and why?
- How many lymph nodes will be removed and what is the lymph node plan?
- Is the surgery open, laparoscopic or robotic?
- Will I need chemotherapy before or after surgery?
- How will eating change, and will I need vitamin B12 injections?
- What is your complication management, including leaks?
Red flags when comparing hospitals
- No endoscopy, staging CT or tumour board review before surgery.
- Surgeon cannot say how many gastrectomies are done each year.
- No dietitian or advice on diet and supplements afterwards.
- No explanation of the join type or leak risk.

Small meals five or six times a day, eaten slowly. The remaining stomach holds less, and your dietitian will guide you.
