About Strabismus (Squint) Surgery

Strabismus surgery adjusts the muscles that move the eye to straighten misaligned eyes. It treats squint in children and adults, improving alignment and, in some cases, depth perception and double vision.

It is done under general anaesthesia in children and often in adults, though adjustable-suture techniques may use local anaesthesia. The surgeon opens the conjunctiva, isolates the eye muscles and weakens, strengthens or repositions them, which changes the direction the eye points. No part of the eye itself is cut, and the stitches dissolve.

Usually a good fit if

  • Constant or frequent squint (esotropia or exotropia) not corrected by glasses.
  • Double vision from muscle imbalance that is stable.
  • Squint causing functional or social difficulty.
  • Stable angle on repeated measurements.

May need to wait if

  • Unstable or changing angle, where waiting may be advised.
  • Untreated amblyopia, which should be addressed first in children.
  • Active eye infection or uncontrolled general illness.
  • Need for further investigation of the underlying cause, such as nerve palsy.
How to prepare
  1. Bring prior eye reports, glasses prescriptions and prism measurements; amblyopia (lazy eye) should be assessed and treated.
  2. Have orthoptic assessment of the angle of squint, double vision and eye movements.
  3. Fast as instructed for general anaesthesia; for children, discuss feeding times with the team.
  4. Review medicines with your doctor; do not stop any yourself.
  5. Plan time off school or work and stay near for early checks.
Questions to ask your surgeon
  1. What type of squint do I or my child have, and which muscles will be operated?
  2. Will adjustable sutures be used?
  3. What is the chance of under- or over-correction and a second operation?
  4. Will surgery restore binocular vision or only improve appearance?
  5. How is double vision after surgery managed?
  6. What follow-up is needed at home?
Red flags when comparing hospitals
  • No orthoptic assessment before surgery.
  • Operating without treating amblyopia in a child.
  • Promising a one-time permanent cure.
  • No plan for follow-up alignment checks.
