About Hip Resurfacing (Birmingham)

Hip resurfacing caps the ball of the thigh bone with a metal shell instead of removing it, and fits a metal socket. It was designed for younger, active people with good bone and hip arthritis.

Through a larger incision than for a standard hip replacement, the surgeon reshapes the head of the femur and covers it with a metal cap, and places a metal cup in the socket. More of your own bone is kept. It is done under spinal or general anaesthesia. Careful positioning of the parts matters a great deal for how the hip performs.

Usually a good fit if

  • Younger, active adults, usually men under 60, with hip arthritis and good bone quality.
  • Large femoral head and normal anatomy that suit a metal resurfacing.
  • Wish to keep a more natural hip and remain active.
  • Normal kidney function.

May need to wait if

  • Women of small build, who have a higher failure rate with metal-on-metal resurfacing.
  • Poor bone quality, osteoporosis, large cysts in the femoral head or avascular necrosis.
  • Kidney disease or metal allergy.
  • Smaller femoral heads or hip dysplasia.
How to prepare
  1. Have X-rays and sometimes CT or MRI to check femoral head bone quality and socket shape, since resurfacing needs healthy bone.
  2. Mention kidney disease, metal allergy and any previous hip surgery, and ask about blood tests for kidney function.
  3. Review blood thinners and anti-inflammatories with your doctor; do not change them yourself.
  4. Practise using crutches and prepare raised seating at home.
  5. Plan clot prevention for the flight home and check fitness to fly.
Questions to ask your surgeon
  1. Am I a suitable candidate for resurfacing rather than a standard hip replacement?
  2. Which implant do you use, and what is its registry track record?
  3. How many resurfacings have you done?
  4. What follow-up tests for metal ions and imaging do I need?
  5. What are the risks of neck fracture, metal reaction or revision?
  6. Can follow-up be done at home?
Red flags when comparing hospitals
  • Surgeon with limited resurfacing experience; patient selection and technique are critical.
  • Using an implant with a poor registry history.
  • Offering resurfacing to women or those with poor bone without discussion.
  • No metal ion monitoring plan.
