About Hip Replacement

Hip replacement replaces a worn or damaged hip joint with a ball-and-socket implant. It is for people whose arthritis or injury causes pain and limited walking that other treatment has not controlled.

The surgeon removes the damaged head of the thigh bone and the worn surface of the socket, then fits a stem, a ball and a socket liner made from metal, ceramic or plastic. Implants may be cemented or uncemented. Surgery is done under spinal or general anaesthesia through a back, side or front approach, and takes about one to two hours.

Usually a good fit if

  • Hip arthritis, avascular necrosis or damage from injury causing pain and stiffness.
  • Pain not controlled by painkillers, injections or physiotherapy.
  • Pain at night or limitations in walking, stairs and sleep.
  • Fit enough for surgery and willing to follow rehabilitation.

May need to wait if

  • Active hip infection or infection elsewhere.
  • Uncontrolled medical conditions such as unstable heart disease or diabetes.
  • Mild symptoms that still respond to non-surgical treatment.
  • Severe bone loss or neurological problems that raise dislocation risk.
How to prepare
  1. Have hip X-rays and sometimes CT to plan implant size, and tell your surgeon about previous hip surgery.
  2. Review blood thinners, anti-inflammatories and diabetes medicines with your doctor; do not change them yourself.
  3. Treat dental problems and skin infections, and control diabetes and blood pressure before surgery.
  4. Practise using a walker or crutches and prepare raised seating at home.
  5. Plan clot prevention for the flight home and check fitness to fly.
Questions to ask your surgeon
  1. Which approach will you use and why: anterior, lateral or posterior?
  2. Which implant and bearing do you use, and what is its track record?
  3. How many hip replacements do you do each year?
  4. What risks of dislocation, infection or leg-length difference apply?
  5. What movement precautions will I have?
  6. What follow-up and X-rays can I do at home?
Red flags when comparing hospitals
  • No X-ray planning or discussion of implant choice.
  • Using an implant with no long-term registry data.
  • No clot prevention plan.
  • No follow-up arrangement for X-rays.
