About Deep-Plane Facelift

A deep-plane facelift lifts the deeper facial tissues together with the overlying skin, to correct jowls, cheek descent and neck laxity with a natural result. It is for adults with moderate to significant ageing changes.

Under general anaesthesia, the surgeon works beneath the muscle layer (SMAS) and releases the retaining ligaments, so the skin and deeper tissues move as one unit. This allows lifting of the midface and jawline with less tension on the skin. It is technically demanding and takes several hours, often with a neck lift.

Usually a good fit if

  • Jowls, midface descent and neck laxity in adults who want a longer-lasting lift.
  • Skin with some elasticity and good bone structure.
  • Good general health and realistic expectations of a natural, refreshed look.

May need to wait if

  • Smoking, uncontrolled high blood pressure or bleeding disorders.
  • Uncontrolled diabetes or conditions that impair wound healing.
  • Unrealistic expectations or minimal ageing, where a lighter procedure may be enough.
How to prepare
  1. Stop smoking and nicotine well before surgery, since the deep-plane dissection depends on a healthy blood supply.
  2. Review blood thinners, aspirin, vitamin E, fish oil and supplements with your doctor; change them only on their instruction.
  3. Control high blood pressure with your doctor, since it increases bleeding risk.
  4. Bring photos of your face when younger, to guide natural results.
  5. Plan about two weeks near the clinic and arrange help at home.
Questions to ask your surgeon
  1. Why is a deep-plane facelift better for me than a SMAS or mini facelift?
  2. How many deep-plane facelifts have you performed, and what is your facial nerve injury rate?
  3. Where will the incisions sit, and will my hairline change?
  4. Will fat grafting, neck work or brow work be added?
  5. What swelling should I expect, and when can I fly home?
  6. What follow-up and revision policy do you offer?
Red flags when comparing hospitals
  • Surgeon has little deep-plane training or cannot explain facial nerve anatomy.
  • Promise of permanent results.
  • No staging or safety discussion for long combined operations.
  • Photos that always show the same tight, pulled look.
