About ERCP (Endoscopic Retrograde Cholangiopancreatography)
ERCP uses an endoscope and X-ray dye to see and treat problems in the bile and pancreatic ducts. It is mainly used to remove bile duct stones, relieve blockages and place stents.
ERCP is done under sedation or general anaesthesia and takes thirty to ninety minutes. A flexible endoscope is passed through the mouth into the first part of the small bowel, where the bile duct opens. A thin catheter enters it, dye is injected for X-ray pictures, and instruments can widen the opening, remove stones, take samples or place a stent.
Usually a good fit if
- Stones in the common bile duct.
- Blocked bile duct from a tumour or stricture needing a stent.
- Bile leak after surgery.
- Cholangitis, infection of the bile duct, needing urgent drainage.
May need to wait if
- Acute pancreatitis not related to a duct stone.
- Clotting problems or blood thinners not reviewed by the doctor.
- Severe heart or lung disease needing optimisation before sedation.
- Anatomy altered by previous surgery, which may require a specialised approach.
How to prepare
- MRCP, ultrasound or CT scan is usually done first to confirm that ERCP is needed.
- Blood tests, including liver function, clotting and amylase.
- Review blood thinners and diabetes medicines with your doctor and change nothing yourself.
- Fast for at least six hours before the procedure.
- Arrange someone to accompany you home and avoid driving after sedation.
Questions to ask your surgeon
- Is ERCP necessary, or can a scan give the answer?
- What is your rate of successful duct access and of pancreatitis afterwards?
- Will you place a pancreatic stent to lower the risk of pancreatitis?
- What happens if the stone cannot be removed?
- How long will a stent stay in, and when must it be replaced?
- Who do I contact if I develop fever or pain at home?
Red flags when comparing hospitals
- ERCP used purely as a diagnostic test when a scan would do.
- Endoscopist with low annual ERCP numbers.
- No fluoroscopy or X-ray facilities.
- No discussion of pancreatitis risk.
- No plan for stent removal or replacement.
