★★★★★ Bypass that saved me, third of Lagos prices
I was quoted $18k in Lagos for a bypass I couldn't afford. Medanta sent an itemized plan for $5,600 including OR, ICU days, and a week of post-op physio. Dr. Menon FaceTimed me twice before I flew. No surprises on the bill.

I'd been managing diabetes and high blood pressure for 12 years when angina started. My cardiologist in Dubai recommended bypass — 4-vessel disease. After getting opinions from three programs, we went with Medanta in Gurgaon. The pre-op…

About CABG (Heart Bypass Surgery)

Coronary artery bypass grafting reroutes blood around blocked heart arteries using healthy vessels taken from elsewhere in the body. It is used for significant coronary disease, particularly when several arteries are affected.

Under general anaesthesia, the surgeon usually opens the chest through the breastbone. Grafts are taken from the chest wall, the leg or the arm and sewn beyond each blockage. This is done with the heart stopped and a heart-lung machine, or on the beating heart in an off-pump approach. The number of grafts depends on how many arteries are narrowed.

Usually a good fit if

  • Blockages in several coronary arteries, or in the left main artery, especially with diabetes or weak heart pumping.
  • Angina not controlled by medicines where stenting is not suitable.
  • Heart attack or coronary disease with other heart surgery needed at the same time.
  • Fitness for open-chest surgery.

May need to wait if

  • Very limited or small-vessel disease better treated by stents or medicines.
  • Severe lung, kidney or brain disease, or recent stroke, raising risk too high.
  • Active infection or uncorrected bleeding disorder.
  • Heart muscle that is scarred beyond benefit from revascularisation.
How to prepare
  1. Complete blood tests, ECG, echocardiogram, chest X-ray and kidney checks, and bring your coronary angiogram or CT scan.
  2. Review aspirin, clopidogrel, blood thinners and diabetes drugs with your doctor, who will tell you which to adjust or stop beforehand.
  3. Ask about leg vein or arm artery harvest sites and how to care for them.
  4. Stop smoking now and practise breathing exercises taught by the physiotherapist.
  5. Plan help at home for the first weeks and a cardiac rehabilitation programme.
Questions to ask your surgeon
  1. Is bypass better than stenting for my angiogram and diabetes?
  2. Which grafts will you use, and will you use arteries?
  3. On-pump or off-pump, and why?
  4. How many CABG operations does your team perform, and what is your mortality at my risk level?
  5. Which medicines and cholesterol targets will I need afterwards?
  6. What cardiac rehabilitation can I join at home?
Red flags when comparing hospitals
  • No heart team discussion between a surgeon and cardiologist.
  • Surgeon unable to explain graft choices.
  • No cardiac intensive care or blood bank on site.
  • Guarantees that bypass will cure heart disease.
  • No plan for secondary prevention and rehabilitation.
