Most repairs are done arthroscopically, with small incisions and a camera, though larger tears may need an open or mini-open approach. The surgeon smooths the bone, places anchors into the humerus, and passes stitches through the tendon to pull it back to its footprint. A sling protects the repair while the tendon heals to the bone.
Pain relief is usually reliable, while strength and range return slowly over six to twelve months. Results depend on the size and age of the tear, the quality of the muscle and tendon, smoking, diabetes, age and keeping to the rehabilitation plan. Large chronic tears have a higher chance of not healing, or of tearing again.
Many people go home the same day or the next. Plan to remain near the hospital for the first wound review, and arrange for help at home, since the sling is worn for around six weeks and the arm is kept supported. Bring your MRI, and book physiotherapy before you travel. Driving and overhead work wait for several months.

- Acute tear in a younger person, where early repair gives the best result.
- Partial tears that have not responded to physiotherapy and injections.
- Good shoulder movement before surgery and commitment to a long rehabilitation.

- Massive, long-standing tears with severe fatty degeneration of the muscle, which may not heal.
- Advanced shoulder arthritis, where a different operation may be needed.
- Smoking or uncontrolled diabetes that may impair healing.
- Not able to follow sling and rehabilitation rules.

- Bring MRI or ultrasound and previous shoulder treatment notes; tear size guides the repair plan.
- Ask your doctor about blood thinners, steroid injections and anti-inflammatories to review before surgery.
- Arrange to sleep in a reclined position, since lying flat is uncomfortable for weeks.
- Have routine blood tests and an anaesthetic review; stop smoking, which weakens tendon healing.
- Organise help at home; you will have one arm in a sling for about six weeks.
