Hypersplenism / Splenomegaly

Hypersplenism is overactivity of an enlarged spleen, which removes too many blood cells from the circulation. Splenomegaly can result from liver cirrhosis, blood disorders, infections or lymphoma.

О hypersplenism / splenomegaly

The spleen enlargement may cause fullness or pain in the upper left abdomen and early satiety. Blood tests show low red cells, white cells or platelets, leading to tiredness, infections or easy bruising. Diagnosis is by examination, ultrasound or CT and blood tests, with further tests to find the underlying cause, which is the key to treatment.

Treatment is aimed at the underlying disease. When cytopenias are severe or the spleen is painful or very large, splenectomy removes it, usually by laparoscopic surgery, giving a quick recovery. Vaccination against pneumococcus, meningococcus and Haemophilus is given before surgery, and long-term infection precautions are needed afterwards. Partial splenic embolisation is an alternative in selected patients.

  • Full blood count with differential and platelet count, showing which cell lines are low, and trends over time.
  • Ultrasound, CT or MRI reports measuring spleen size and checking the liver, portal vein and any varices.
  • Liver function, hepatitis B and C status and, if relevant, bone marrow biopsy and blood film results.
  • The underlying cause, for example cirrhosis with portal hypertension, a blood disorder or infection.
  • Vaccination record for pneumococcus, meningococcus and Haemophilus influenzae, which are usually requested before splenectomy.

  • Splenectomy is advised but the underlying cause, such as liver cirrhosis, has not been fully investigated.
  • You have cirrhosis with portal hypertension and are choosing between splenectomy, embolisation and watchful monitoring.
  • Low counts are mild and you have no bleeding or infection problems.
  • A lymphoma or blood cancer has not been ruled out.

  1. What is causing my enlarged spleen and low blood counts?
  2. Is the spleen the main reason my counts are low?
  3. Do I need treatment, or is monitoring enough?
  4. Would partial splenic embolisation be an option instead of removal?
  5. What vaccines and precautions do I need if my spleen is removed?
  6. What are the risks of clots and infection afterwards?

Outlook depends mainly on the cause, such as cirrhosis, blood disease or infection. Removing or reducing the spleen usually raises blood counts, but does not treat the underlying disease. After splenectomy there is lifelong increased risk of serious infection, so vaccination, antibiotics advice and prompt care for fever are essential. There is also a risk of portal vein clots. Many people with mild hypersplenism need monitoring only.

Do I need my spleen removed?

Not always. Surgery is for severe low counts causing bleeding or infections, or when treating the cause fails. Mild cases are monitored.

Can I live without a spleen?

Yes, but you are more prone to serious infection, so vaccinations, preventive antibiotics as advised, and prompt care for fever are essential.

Are there alternatives to removal?

Partial splenic embolisation reduces the spleen's size and can improve blood counts in selected patients, with its own risks such as pain and infection.

Clinical context

Working up hypersplenism / splenomegaly properly

A condition like hypersplenism / splenomegaly is rarely well-served by jumping straight to treatment shopping. The sequence below is what a хирургия жкт specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or хирургия жкт specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
