О liver cirrhosis (decompensated)

Typical signs include a swollen abdomen (ascites), yellow skin and eyes, vomiting blood, black stools, leg swelling and episodes of confusion known as hepatic encephalopathy. Diagnosis rests on blood tests, ultrasound or elastography and endoscopy to look for varices, with scoring systems such as MELD used to judge severity. Once complications appear, the outlook is considerably shorter without a transplant.

Care focuses on the underlying cause, such as stopping alcohol or treating hepatitis, plus diuretics, a low-salt diet, beta-blockers or band ligation for varices, and lactulose for confusion. Procedures such as paracentesis or TIPS may be needed. Liver transplantation, from a deceased or living donor, is the definitive treatment, and people sometimes look abroad when donor organs are scarce or waiting times are long.

  • Recent liver blood tests, including bilirubin, albumin, INR, creatinine and sodium, which feed into MELD scoring.
  • Liver ultrasound or CT/MRI reports, including checks for portal vein clots and for any liver lesion that could be cancer.
  • Endoscopy report showing oesophageal or gastric varices, and any record of past bleeding or banding.
  • Cause of cirrhosis: hepatitis B and C results, alcohol history, autoimmune or metabolic work-up, and any viral treatment given.
  • A history of complications: ascites drainage, infections of the abdominal fluid, confusion episodes, hospital admissions and current medicines.
  • Cardiac and lung assessments such as echocardiogram, usually requested before transplant evaluation.

  • You have had your first major complication, such as variceal bleeding, ascites or confusion, and nobody has discussed transplant assessment.
  • You have been told you are not a transplant candidate and want the reasons, especially if they relate to age, kidney function or a liver lesion.
  • A living donor is being considered and you want the donor's risks and the graft size explained independently.
  • Your MELD score is rising despite treatment, or you are being admitted repeatedly.

  1. What is my MELD score and Child-Pugh class, and how quickly are they changing?
  2. Is the cause of my cirrhosis still active, and can it be treated to slow the damage?
  3. Should I be assessed for transplantation now, and what would make me a poor candidate?
  4. If a relative offers to donate part of their liver, how are donor safety and graft size assessed?
  5. How are my varices, ascites and confusion being monitored, and what warning signs mean I must go to hospital at once?
  6. Is it safe for me to fly, and what should I carry or arrange for the journey?

Decompensated cirrhosis carries a serious outlook without transplantation, and survival falls with each further complication. Liver transplantation can restore a good quality of life for many people, and most recipients who survive the first year do well for years. Outcome depends on the cause, MELD score, kidney function, infection, and whether liver cancer is present. Treating the underlying cause, such as hepatitis or alcohol use, can sometimes stabilise the liver.

Is a liver transplant the only treatment?

Transplant is the only treatment that replaces a failing liver. Medicines, endoscopic treatment and procedures can control complications such as fluid and bleeding, and treating the underlying cause can sometimes stabilise the liver, but they do not reverse advanced scarring.

Can part of a relative's liver be used?

Yes, in a living donor liver transplant a portion of a healthy donor's liver is used and regrows. The donor needs thorough testing and takes on real surgical risk, so the decision is made carefully.

Is it safe to travel with decompensated cirrhosis?

It depends on how stable you are. Fluid, bleeding risk, confusion and infection all matter. Ask your liver specialist whether you are fit to fly and what to carry, and plan for medical support at the destination.

Clinical context

Working up liver cirrhosis (decompensated) properly

A condition like liver cirrhosis (decompensated) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a трансплантация органов specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or трансплантация органов specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
