Sobre Hirschsprung Disease (Pull-Through)
The surgeon removes or bypasses the affected segment, which cannot relax and push stool along, and brings healthy bowel down to just above the anus. Common techniques are the Soave, Swenson and Duhamel operations, and many are done through the anus, with laparoscopic help, or in one stage. Some babies first need a colostomy and a second operation later.
Most children do well and pass stool through the anus, but bowel function takes time and varies. Constipation, soiling or frequent stools can occur, and some develop enterocolitis, an infection of the bowel that needs urgent attention. Results depend on the length of the affected bowel, the technique, associated conditions such as Down syndrome and the surgeon's experience.
Plan for a hospital stay of about a week, with feeding resumed gradually. Parents are taught anal dilatation and how to recognise enterocolitis, with fever, swelling and watery stool as warning signs. Stay for the first follow-up, bring your child's biopsy report and imaging, and have a paediatric surgeon you can contact at home.
Geralmente indicado se
- Babies and children with biopsy-proven Hirschsprung disease and constipation or enterocolitis.
- Children stable on washouts or a stoma and ready for definitive surgery.
- Children whose nutrition and general health are suitable for anaesthesia.
- Families able to attend regular follow-up.
Pode ser necessário esperar se
- Active enterocolitis or sepsis, which must be stabilised first.
- Poor nutrition or low weight that needs correction.
- Total colonic aganglionosis, which needs specialist planning.
- Anaesthetic risks from heart or other conditions.
Como se preparar
- Bring the rectal biopsy report, contrast enema images and any earlier colostomy records.
- Check your child's nutrition and weight; infections of the bowel (enterocolitis) should be treated first.
- Ask whether bowel washouts are needed before surgery to keep the colon empty.
- Complete blood tests and anaesthetic review, including checks for associated conditions such as Down syndrome.
- Learn about colostomy care and washouts if your child has them.
Perguntas para fazer ao seu cirurgião
- How long is the abnormal segment, and what is your plan for the pull-through?
- Will you use a transanal, laparoscopic or open approach?
- Do we need a stoma first, and how long would it stay?
- How often does enterocolitis occur after surgery, and how can I recognise it?
- What will bowel function be like in the long run?
- What follow-up and bowel management can be done at home?
Sinais de alerta ao comparar hospitais
- Surgery planned without a rectal biopsy confirming the diagnosis.
- No paediatric surgeon or neonatal and paediatric intensive care.
- The team does not explain the signs of enterocolitis to parents.
- Pathology unable to confirm the transition zone during surgery.
