Sobre Descompressão de malformação de Chiari

Through an incision at the back of the head and neck, the surgeon removes a small piece of bone at the skull base, and often part of the first vertebra. The covering of the brain (dura) may be opened and widened with a patch to allow spinal fluid to flow freely. Variations differ in whether the dura is opened.

Headaches brought on by coughing or straining often improve, and numbness or weakness may stabilise or improve. Outcomes depend on how long symptoms have lasted, whether there is a syrinx in the spinal cord and the technique used. Some symptoms, especially long-standing nerve damage, may improve only partly.

Plan for around five days in hospital and several weeks of neck-conscious rest before a long journey. Bring your MRI images, including spine scans if a syrinx is present. Arrange neurosurgical follow-up at home and know the warning signs, such as a fluid leak at the wound or a fever with severe headache.

Geralmente indicado se

  • Symptomatic Chiari malformation with cough-related headaches, neck pain or balance trouble.
  • Chiari malformation with a syrinx in the spinal cord.
  • Swallowing, breathing or nerve symptoms linked to brainstem compression.
  • Progressive symptoms despite medical management.

Pode ser necessário esperar se

  • Chiari findings on MRI without symptoms are usually observed.
  • Hydrocephalus that needs treating first or alongside.
  • Severe medical illness making surgery unsafe.
  • Active infection or uncorrected clotting problems.
Como se preparar
  1. Bring your brain and spine MRI on disc, including any flow study, and notes about headaches or other symptoms.
  2. Review blood thinners and anti-inflammatory medicines with your doctor, who will instruct changes.
  3. Expect blood tests and a chest and heart check before anaesthesia.
  4. Tell the team if you have a syrinx, scoliosis or hydrocephalus, as these affect planning.
  5. Plan to stay within reach of the hospital for the first weeks, as a spinal fluid leak needs prompt attention.
Perguntas para fazer ao seu cirurgião
  1. Is my headache likely to be from the Chiari malformation, and how likely is surgery to help?
  2. Will you open the dura and use a patch, and why?
  3. How many Chiari decompressions do you perform each year?
  4. What are the risks of spinal fluid leak or meningitis, and how are they managed?
  5. If I have a syrinx, what do you expect to happen to it?
  6. What follow-up MRI and checks are needed at home?
Sinais de alerta ao comparar hospitais
  • Surgery recommended for MRI findings alone, with no matching symptoms.
  • No explanation of the different decompression techniques.
  • No neurosurgical intensive care for complications.
  • Promise of complete relief of all symptoms.
