Sobre unexplained infertility
O que você precisa saber
There are no symptoms beyond not conceiving. The diagnosis is made by exclusion after hormone tests, an assessment of the tubes and womb, and a semen analysis. Outcomes depend strongly on the woman's age and how long the couple has been trying; some conceive naturally, but chances fall with age, so the timing of treatment matters.
Younger couples are often advised to keep trying with lifestyle guidance first. Intrauterine insemination, with or without mild ovarian stimulation, is a common next step. If this fails, or the woman is older, IVF with ICSI is usually offered. Couples sometimes seek treatment abroad for access to timely care or particular laboratory techniques.
Avaliação pré-viagem
Exames que devem já estar prontos
- A recent semen analysis report for the male partner, ideally with more than one result if available.
- Hormone tests for the female partner, such as AMH, FSH, LH, estradiol and thyroid function, and records showing ovulation.
- Tubal assessment reports, such as hysterosalpingogram, sonohysterogram or laparoscopy findings.
- A pelvic ultrasound report and details of any previous fertility treatment, with drugs, doses and outcomes for each cycle.
- The woman's age, how long you have been trying, and any history of pregnancy, miscarriage or pelvic surgery.
Segunda opinião
Quando buscar uma segunda avaliação
- You have had a diagnosis of unexplained infertility but one key test, such as tubal patency or semen analysis, was done only once or long ago.
- You have failed several IUI cycles and are told to go straight to IVF, or the reverse.
- The woman is over 35 and has been advised to keep waiting rather than start treatment.
- You have had multiple failed IVF cycles and want a review of what may have been missed.

Perguntas para fazer
- Have all the standard tests been done, and are any worth repeating or adding?
- Given our ages and time trying, should we try IUI first or move directly to IVF?
- How many IUI cycles are reasonable before changing approach?
- Would ICSI offer any advantage for us if the semen result is normal?
- What is our realistic chance of a live birth per cycle, and how does it change with age?
- What are the risks of ovarian stimulation and of multiple pregnancy?
Perspectiva
Prognóstico realista
Many couples with unexplained infertility do conceive, either naturally over time or with treatment. The woman's age is the strongest factor, and the chance of a live birth per cycle falls as age rises, particularly after 35. IUI gives modest per-cycle success rates, while IVF generally gives higher rates per cycle. Duration of infertility, ovarian reserve and previous pregnancies also influence outcome. No single treatment guarantees pregnancy.

Does unexplained infertility mean there is no problem?
It means standard tests found no cause, but there may be subtle problems with egg quality, sperm function or implantation that current tests do not detect. It is a real diagnosis and treatment can still work.
Should we try IUI or go straight to IVF?
It depends on the woman's age, how long you have been trying and previous treatment. IUI is simpler but has lower success per cycle; IVF is more intensive and has higher success. Your specialist will guide the order.
Can we still conceive naturally?
Yes, some couples with unexplained infertility conceive without treatment, especially if the woman is younger and the time trying is short. The chance falls with age and longer duration.
How long does a treatment cycle take?
An IUI cycle takes about two weeks from stimulation to pregnancy test. An IVF or ICSI cycle usually takes four to six weeks, including stimulation, egg collection, embryo transfer and the wait for a test.
Clinical context
Working up unexplained infertility properly
A condition like unexplained infertility is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilidade e fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or fertilidade e fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
