Sobre tubal factor infertility

O que você precisa saber

There are often no symptoms. Past pelvic infection, endometriosis, previous surgery or an ectopic pregnancy are common causes. It is diagnosed by a hysterosalpingogram (dye X-ray), a sonographic dye test or laparoscopy. A blocked tube holding fluid (hydrosalpinx) can also reduce IVF success. Other fertility factors are checked at the same time.

IVF, usually with ICSI when sperm quality needs support, bypasses the tubes and is the most common treatment. Tubal surgery can help in selected women with mild damage, or after sterilisation reversal by recanalisation. A damaged fluid-filled tube is often removed or blocked before IVF. Many couples travel abroad for IVF when local treatment is limited, and age and ovarian reserve strongly influence success.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Hysterosalpingogram, saline sonography or laparoscopy reports showing whether one or both tubes are blocked, and where.
  • Ovarian reserve tests, such as AMH, antral follicle count and FSH, which strongly affect the options.
  • Semen analysis of the male partner.
  • A history of pelvic infection, endometriosis, ectopic pregnancy, sterilisation or abdominal surgery.
  • Ultrasound for hydrosalpinx (a fluid-filled blocked tube), which can reduce IVF success.

Segunda opinião

Quando buscar uma segunda avaliação

  • IVF has been advised and a swollen, fluid-filled tube (hydrosalpinx) has not been discussed.
  • Tubal reversal is being considered after sterilisation, and you want to compare it with IVF.
  • You are over 35 or have low ovarian reserve and time matters.
  • Previous IVF cycles failed and the womb and tubes have not been re-examined.

Perguntas para fazer

  1. Which tubes are blocked, and where?
  2. Would tubal surgery, IVF or ICSI give me the better chance?
  3. Does a hydrosalpinx need to be treated before an embryo transfer?
  4. How does my age and ovarian reserve change the likely success?
  5. What are the risks of ectopic pregnancy and multiple pregnancy?
  6. How many cycles should I plan for before reviewing the approach?

Perspectiva

Prognóstico realista

IVF bypasses blocked tubes entirely, and for tubal factor it works well, particularly in women under 35, with success per cycle falling with age. Tubal surgery can restore natural conception in selected cases, such as reversal of sterilisation or mild damage, but carries a higher risk of ectopic pregnancy. A hydrosalpinx should usually be treated before IVF to protect results. Your age and egg reserve are the most important factors.

04 · Perguntas frequentes

Dúvidas comuns sobre tubal factor infertility

Can blocked tubes be opened?

Sometimes. Tubal surgery can help certain blockages, but many people choose IVF, which bypasses the tubes, particularly when damage is severe or time is limited.

Does a fluid-filled tube affect IVF?

Yes. A hydrosalpinx can reduce implantation, so surgical removal or blocking of the tube is often advised before embryo transfer.

Can I get pregnant with one blocked tube?

Yes, if the other tube is healthy and open, natural conception is still possible, though it may take longer.

Clinical context

Working up tubal factor infertility properly

A condition like tubal factor infertility is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilidade e fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilidade e fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
