Triple-Negative Breast Cancer
Triple-negative breast cancer is a breast cancer that lacks oestrogen receptors, progesterone receptors and HER2. Hormone and HER2-targeted treatments therefore do not work.

Sobre triple-negative breast cancer
O que você precisa saber
It often shows as a lump in the breast and tends to grow quickly, affecting younger women and those with BRCA1 gene changes more often. Diagnosis uses mammography, ultrasound and a needle biopsy, with receptor testing confirming the type, and scans for spread. It carries a higher early risk of returning than other types, but responds well to chemotherapy.
Treatment combines surgery, chemotherapy and, for many, immunotherapy with a PD-1 drug such as pembrolizumab. Chemotherapy is often given before surgery to shrink the tumour. Surgery is either breast-conserving or mastectomy with sentinel node biopsy, and radiotherapy often follows. Genetic testing guides decisions. People sometimes travel abroad for immunotherapy or second opinions, but delays in starting treatment should be avoided.
Avaliação pré-viagem
Exames que devem já estar prontos
- Biopsy pathology confirming grade, Ki-67 and receptor status (ER, PR, HER2), ideally with a repeat HER2 test if borderline.
- Breast MRI, mammogram and ultrasound with images, showing tumour size and spread to the axilla.
- Staging scans such as CT or PET-CT, for larger or node-positive disease.
- Genetic testing results for BRCA1 and BRCA2 and other genes, which affect surgery and drug choices.
- A full list of chemotherapy and immunotherapy given, with doses, dates and any reactions.
Segunda opinião
Quando buscar uma segunda avaliação
- Surgery has been scheduled first, without discussing chemotherapy before the operation.
- BRCA testing has not been done and may affect your surgical and drug choices.
- You had chemotherapy before surgery and cancer remains in the removed tissue, which may qualify you for more treatment.
- The cancer has spread, or you want to know whether immunotherapy is suitable for your tumour.

Perguntas para fazer
- What are the stage and grade of my cancer, and are the receptor results confirmed?
- Should chemotherapy and immunotherapy be given before surgery?
- Do I need a genetic test, and how would the result change treatment?
- Is breast conservation possible, or is mastectomy advisable?
- What happens if cancer remains after chemotherapy?
- What follow-up and imaging will I need, and when is the risk of recurrence highest?
Perspectiva
Prognóstico realista
Triple-negative breast cancer has a higher chance of returning than other types, mostly in the first three to five years, but many patients, particularly with early-stage disease, are cured. Response to chemotherapy before surgery is a strong predictor: patients with no remaining cancer have a much better outlook. Immunotherapy and newer drugs have improved results. Outlook depends on stage, node involvement and response to treatment, and BRCA carriers need tailored advice.

Why is triple-negative breast cancer treated differently?
Because it lacks oestrogen, progesterone and HER2 receptors, hormone therapy and HER2 drugs do not work, so chemotherapy, and in many cases immunotherapy, are the main systemic treatments.
Should I have genetic testing?
Usually yes. A significant share of triple-negative cancers are linked to BRCA1 or BRCA2 changes, which affect treatment and family risk.
Is chemotherapy given before or after surgery?
Often before, for larger or node-positive tumours, because response to treatment guides further therapy. Smaller tumours may have surgery first.
Clinical context
Working up triple-negative breast cancer properly
A condition like triple-negative breast cancer is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or oncologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
