Sobre thymoma / thymic cancer
O que você precisa saber
Many thymomas are found by chance on a chest X-ray or CT. Larger ones cause cough, chest pain, breathlessness or swelling of the face and neck. About a third of patients have myasthenia gravis, with drooping eyelids and muscle weakness. CT or MRI shows the tumour, a biopsy may be needed, and staging decides the plan. Most grow slowly.
Complete surgical removal of the thymus and tumour is the main treatment, through a breastbone incision or by keyhole or robotic surgery for early tumours. Radiotherapy is added if removal is incomplete or the tumour has spread locally, and chemotherapy is used for advanced disease. Long-term follow-up scans are needed, since recurrence can be late. Patients sometimes seek specialist thoracic centres abroad.
Avaliação pré-viagem
Exames que devem já estar prontos
- CT scan of the chest with contrast, with images, showing the size of the mass and whether it touches the heart, great vessels or lung.
- Biopsy or pathology reports with WHO type, and Masaoka-Koga stage if surgery has been done.
- Antibodies for myasthenia gravis (acetylcholine receptor) and a neurological assessment of muscle weakness.
- PET-CT, if requested, to look for spread, and lung function tests before surgery.
- A record of medicines such as pyridostigmine or steroids, and any breathing or swallowing difficulty.
Segunda opinião
Quando buscar uma segunda avaliação
- Surgery has been advised without discussing whether the tumour is fully removable.
- You have myasthenia gravis symptoms, which need specialist planning before an operation.
- The tumour is large or invading nearby structures and chemotherapy before surgery is being discussed.
- A thymoma has returned after surgery, and you want to compare options.

Perguntas para fazer
- What type and stage of thymoma is this, and is it likely to be completely removable?
- Would keyhole, robotic or open surgery be best for my tumour?
- Will I need radiotherapy or chemotherapy after the operation?
- How does my myasthenia gravis affect the risk of surgery?
- What follow-up scans will I need, and for how many years?
- What is the chance of recurrence?
Perspectiva
Prognóstico realista
Thymomas are generally slow-growing, and complete surgical removal is the most important factor for a good outcome. Early-stage tumours that are fully removed have long survival rates, while invasive tumours and thymic carcinoma carry a higher risk of recurrence and need additional treatment. Recurrences can occur many years later, so long-term follow-up with scans is advised. Myasthenia gravis often improves, but not always, after thymectomy.

Is a thymoma cancer?
It is a tumour of the thymus that ranges from slow-growing to invasive. Even non-invasive thymomas are treated as potentially malignant and removed, while thymic carcinoma is a more aggressive cancer.
Why is myasthenia gravis linked to thymoma?
A substantial number of people with thymoma have myasthenia gravis, an autoimmune muscle weakness. It needs control before surgery, and symptoms may continue or change afterwards.
Can a thymoma come back?
Yes, sometimes years later, particularly after incomplete removal or with invasive tumours. Follow-up CT scans are usually continued for many years.
Clinical context
Working up thymoma / thymic cancer properly
A condition like thymoma / thymic cancer is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or oncologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
