Splenic Cyst / Tumour

A splenic cyst or tumour is a fluid-filled or solid lesion in the spleen, the organ under the left ribs that filters blood. Many are found by chance on a scan done for another reason.

Sobre splenic cyst / tumour

O que você precisa saber

Small lesions usually cause no symptoms. Larger ones can cause left upper abdominal pain, a feeling of fullness or, rarely, rupture. Ultrasound, CT and MRI show whether a lesion is a simple cyst, a hydatid cyst, a benign tumour such as a haemangioma or something more concerning. Most simple cysts stay stable for years and are only monitored.

Small, symptom-free lesions are watched with repeat imaging. Cysts causing symptoms or growing may be drained or have part of the spleen removed, preserving its immune function where possible. Laparoscopic splenectomy is used for large lesions, suspected cancer or when the spleen cannot be saved, followed by vaccinations against certain infections. Treatment abroad is mostly considered when specialist laparoscopic or spleen-sparing surgery is not available locally.

Avaliação pré-viagem

Exames que devem já estar prontos

  • CT or MRI of the abdomen with contrast, with the original images, showing the size, wall, internal features and any calcification.
  • Blood tests including a full blood count, and hydatid (echinococcal) serology if you live in or have lived in an area where it is common.
  • Any previous abdominal imaging, so the lesion can be compared over time to show whether it is growing.
  • A history of abdominal injury, infection, lymphoma or known cancer elsewhere, written down with dates.
  • A record of vaccinations, because removal of the spleen usually needs specific vaccines beforehand.

Segunda opinião

Quando buscar uma segunda avaliação

  • The report mentions a solid component or growth over time, and nobody has explained whether it could be lymphoma or a metastasis.
  • Total removal of the spleen has been advised for a small cyst that is not causing symptoms.
  • A parasitic (hydatid) cyst is suspected, since the wrong approach can spill cyst contents.
  • You are being offered surgery but have not been asked about spleen-sparing options.

Perguntas para fazer

  1. Is this a true cyst, a parasitic cyst, or a solid tumour, and how sure are you from the scans?
  2. Does it need treatment at all, or can it be watched with repeat imaging?
  3. Can part of the spleen be saved instead of removing all of it?
  4. If the spleen is removed, which vaccines and precautions will I need for life?
  5. What symptoms should make me seek urgent care, such as sudden left-sided pain?
  6. How often will I need follow-up scans afterwards?

Perspectiva

Prognóstico realista

Most simple splenic cysts are harmless and cause no problems for years, and small ones are often only watched. Larger or symptomatic cysts are treated successfully by surgery. For solid tumours, the outlook depends on what the lesion is: benign tumours are cured by removal, while lymphoma or metastatic disease depends on the underlying cancer. After spleen removal most people live normally but have a lifelong higher risk of serious infection.

Does a splenic cyst always need to be removed?

No. Small cysts without symptoms are often just watched with repeat scans. Treatment is usually considered for large cysts, pain, growth over time, infection, or when a tumour or parasitic cyst cannot be excluded.

Can I live normally without a spleen?

Yes, most people do. The main concern is a higher lifelong risk of serious infection, so vaccinations, sometimes preventive antibiotics, and prompt treatment of fevers are advised. Where possible, surgeons try to preserve part of the spleen.

Can a splenic cyst come back after treatment?

After complete removal of a cyst wall or the spleen, it does not return. After simple drainage, some cysts refill, which is why surgery is often preferred for larger ones.

Clinical context

Working up splenic cyst / tumour properly

A condition like splenic cyst / tumour is rarely well-served by jumping straight to treatment shopping. The sequence below is what a cirurgia gastrointestinal specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or cirurgia gastrointestinal specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
