Allergic Rhinitis (Refractory)

Refractory allergic rhinitis is nasal allergy, with blocked nose, sneezing and itching, that stays troublesome despite sprays and antihistamines. Nasal blockage is often the main complaint.

Sobre allergic rhinitis (refractory)

O que você precisa saber

Symptoms include sneezing, runny and itchy nose, itchy eyes and persistent blockage, and they can affect sleep, concentration and work. Diagnosis involves history and nasal examination, and skin prick or blood allergy tests identify triggers such as pollen, dust mites or pets. A deviated septum, polyps or sinus disease can add to blockage and are checked with nasal endoscopy.

Avoiding triggers, saline rinses, daily steroid nasal sprays, non-sedating antihistamines and combination sprays come first. Allergen immunotherapy, as tablets or injections, can reduce allergy over several years. When enlarged turbinates continue to block the nose despite medicines, turbinoplasty reduces their size and improves airflow, though it does not cure the allergy itself. Treatment abroad is rarely needed.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Allergy test results, either skin prick testing or blood-specific IgE, identifying the triggers.
  • A list of treatments tried (steroid sprays, antihistamines, saline rinses) with doses, duration and response.
  • Nasal endoscopy report and, if available, CT of the sinuses to check for septal deviation, polyps or sinusitis.
  • A symptom diary with seasonality, triggers and effect on sleep.
  • Asthma history and lung function results, since the two conditions often occur together.

Segunda opinião

Quando buscar uma segunda avaliação

  • Surgery is advised before allergy testing or a proper trial of sprays used correctly.
  • Your blocked nose is worse on one side, which may signal a deviated septum or other cause.
  • You have asthma and have not been assessed for immunotherapy.
  • Symptoms are not seasonal and may be non-allergic rhinitis, which responds differently.

Perguntas para fazer

  1. Which allergens trigger my symptoms, and can I reduce exposure?
  2. Am I using my nasal spray correctly and for long enough?
  3. Is allergen immunotherapy suitable for me?
  4. Is the blockage due to swollen turbinates, a deviated septum or both?
  5. What does turbinate reduction involve, and how long does the benefit last?
  6. Could my symptoms be due to something other than allergy?

Perspectiva

Prognóstico realista

Allergic rhinitis is a long-term condition that can be controlled but not cured by medicine. Immunotherapy can give lasting benefit in selected people. Turbinate reduction relieves blockage in most patients, though the effect may wear off over years. Outcomes depend on identifying triggers, treating coexisting problems like septal deviation and sticking to treatment.

04 · Perguntas frequentes

Dúvidas comuns sobre allergic rhinitis (refractory)

Will surgery cure my allergy?

No. Turbinate surgery improves the airflow by reducing swollen tissue, but it does not remove the allergy, and sneezing and itching may continue.

Can allergies be treated at the source?

Yes. Allergen immunotherapy, by tablets or injections, gradually reduces sensitivity over three to five years in suitable patients.

How long does turbinate reduction last?

Many people get years of benefit, but the turbinates can swell again, especially if the allergy is not well controlled.

Are nasal sprays safe long-term?

Steroid sprays are safe for long-term use at standard doses. Decongestant sprays, however, should not be used for more than a few days.

Clinical context

Working up allergic rhinitis (refractory) properly

A condition like allergic rhinitis (refractory) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a otorrinolaringologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or otorrinolaringologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
