Sobre recurrent pregnancy loss

O que você precisa saber

Couples face repeated early losses, and sometimes later ones. Evaluation looks for chromosome changes in either parent, antiphospholipid syndrome, uterine abnormalities such as a septum, thyroid disease and diabetes. Tests include blood tests, genetic testing, and pelvic ultrasound or hysteroscopy. Even with no cause found, the chance of a successful future pregnancy is generally good with supportive care.

Treatment depends on the cause: low-dose aspirin and heparin for antiphospholipid syndrome, surgery for a uterine septum, and control of thyroid disease and diabetes. Where one of the couple carries a balanced chromosome rearrangement, IVF with preimplantation genetic testing (PGT-SR) can select unaffected embryos, and PGT-A is sometimes used although its benefit is debated. Emotional support is important. Couples sometimes travel abroad for genetic testing.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Karyotype results for both partners, and the genetic results of any pregnancy tissue tested after a miscarriage.
  • Antiphospholipid antibody tests (lupus anticoagulant, anticardiolipin, beta-2 glycoprotein), repeated after 12 weeks if positive.
  • Pelvic ultrasound or hysteroscopy findings showing uterine shape, fibroids, polyps or a septum.
  • Thyroid function, HbA1c and prolactin results, and any ultrasound reports of earlier pregnancies.
  • Dates, gestation and outcomes of each pregnancy loss, including any treatment given.

Segunda opinião

Quando buscar uma segunda avaliação

  • No investigations were done after your second or third loss, and you are told nothing can be done.
  • Immune therapies or steroids are recommended without clear evidence.
  • Your losses occurred late in the second trimester, which suggests different causes such as cervical weakness.
  • Embryo screening (PGT-A) is suggested, and you want to know the extent to which it helps compared with natural conception.

Perguntas para fazer

  1. Which tests do I need, and have the key causes been checked?
  2. What does my partner's or my chromosomal result mean for future pregnancies?
  3. Do I need aspirin or heparin, and in which situation is it proven to help?
  4. Is PGT-A or PGT-SR appropriate for us, and what are its limits?
  5. What monitoring should I have in my next pregnancy?
  6. What are our realistic chances of a successful pregnancy?

Perspectiva

Prognóstico realista

The outlook is better than many couples fear. Even after several losses, most women eventually have a live birth, especially when no cause is found and the woman is under 35. Chances fall with age and number of losses. Treatable causes such as antiphospholipid syndrome, a uterine septum or thyroid disease should be dealt with, and supportive care in early pregnancy improves confidence.

04 · Perguntas frequentes

Dúvidas comuns sobre recurrent pregnancy loss

Is it my fault?

No. Most miscarriages occur because of random chromosome problems in the embryo and are not caused by anything a mother did.

Will I be able to have a baby?

Most couples do. Even after three losses, the chance of a live birth in a future pregnancy is high, though lower with age.

Does PGT-A guarantee a healthy pregnancy?

No. It can lower the chance of transferring an embryo with chromosome abnormalities, but it does not guarantee success and requires IVF.

When should we start testing?

Testing is usually offered after two or three losses, and earlier if you are older or had a late loss.

Clinical context

Working up recurrent pregnancy loss properly

A condition like recurrent pregnancy loss is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilidade e fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilidade e fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
