Primary Sclerosing Cholangitis (PSC)
Primary sclerosing cholangitis (PSC) is a chronic disease in which the bile ducts become inflamed and scarred, often alongside inflammatory bowel disease. It most often affects men aged 30 to 50.

Sobre primary sclerosing cholangitis (psc)
O que você precisa saber
Symptoms include fatigue, itching, jaundice and episodes of bile duct infection, though many people are diagnosed from abnormal liver tests alone. MRCP, a special MRI of the bile ducts, shows the typical beaded narrowing. Colonoscopy is advised because of the link with ulcerative colitis, and there is a raised risk of bile duct cancer. The disease slowly progresses to cirrhosis over many years.
No medicine has been proven to halt the disease. Care focuses on treating itching, infections and vitamin deficiency, and on endoscopic dilation of tight bile duct strictures. Regular surveillance looks for cancer. Liver transplantation is the only cure for advanced disease or recurrent cholangitis, and living donor transplant is sometimes possible. Patients sometimes seek treatment abroad when waiting times for a donor liver are long.
Avaliação pré-viagem
Exames que devem já estar prontos
- MRCP (MRI of the bile ducts) images and report showing strictures and the extent of duct disease.
- Liver blood tests over time, including alkaline phosphatase, bilirubin and IgG4 to exclude look-alike conditions.
- Colonoscopy and biopsy results, since most patients have ulcerative colitis or Crohn's disease.
- Elastography or biopsy showing liver scarring, and any CA 19-9 or imaging checks for bile duct cancer.
- Transplant assessment details: MELD score, history of cholangitis episodes, blood group and donor information.
Segunda opinião
Quando buscar uma segunda avaliação
- You have a dominant stricture and are offered repeated stenting, and you want to know about surgical or other options.
- A suspicious mass or rising CA 19-9 raises concern about bile duct cancer.
- Transplant is advised but you have not been assessed for living donor options.
- Your diagnosis is uncertain because MRCP is equivocal or IgG4 is raised.

Perguntas para fazer
- How far has the disease progressed, and is it affecting the large or small bile ducts?
- What is my risk of bile duct cancer, and how will you screen for it?
- Should I have regular colonoscopy as well?
- Is there a role for endoscopic treatment of my strictures?
- When should I be listed for transplant, and does a living donor change the timing?
- What symptoms mean infection of the ducts and need urgent care?
Perspectiva
Prognóstico realista
PSC progresses slowly but steadily in most people, and median transplant-free survival is often reported at roughly 15 to 20 years from diagnosis. Complications include recurrent duct infections and a raised risk of bile duct and colon cancer. Liver transplant gives good long-term results, though PSC can recur in the new liver in a proportion of patients.

Is there a medicine that cures PSC?
No medicine has been proven to stop the progression. Treatment manages symptoms, strictures and complications, and transplant is the definitive option.
Why do I need a colonoscopy?
PSC is closely linked with inflammatory bowel disease, which raises the risk of colon cancer, so regular colonoscopy is usually advised.
Can PSC come back after transplant?
Yes, in some patients, though not all. Regular follow-up detects it.
Can I fly with PSC?
Usually yes if you are well. If you have had recent bile duct infections, discuss travel plans and carry your reports and a plan for fever.
Clinical context
Working up primary sclerosing cholangitis (psc) properly
A condition like primary sclerosing cholangitis (psc) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a transplante de órgãos specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or transplante de órgãos specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
