Polycystic Kidney Disease (PKD)

Polycystic kidney disease (PKD) is an inherited condition in which fluid-filled cysts gradually enlarge both kidneys and can lead to kidney failure. The dominant form usually appears in adulthood.

Sobre polycystic kidney disease (pkd)

O que você precisa saber

Many people have no symptoms early on. Over time they may develop high blood pressure, flank or back pain, blood in the urine, kidney stones and urinary infections. Cysts can also form in the liver, and some people have brain aneurysms. Diagnosis is made by ultrasound, CT or MRI with family history, and genetic testing can help. Kidney function declines gradually, often reaching failure in the fifties or sixties.

Treatment aims to protect kidney function: blood pressure control, plenty of fluids, and in selected patients tolvaptan to slow cyst growth. Infections, pain and stones are treated as they arise. When kidney failure arrives, dialysis or a kidney transplant is needed, and a living donor transplant generally gives the best long-term results. Patients sometimes travel abroad when no suitable donor or transplant access is available locally.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Latest kidney function: serum creatinine, eGFR trend over several years, and urine protein tests.
  • Ultrasound, CT or MRI reports with total kidney volume if measured, plus liver cyst findings.
  • Blood pressure records, since hypertension is common and speeds progression.
  • Family history, any genetic test results, and details of relatives who needed dialysis or had brain aneurysms.
  • If transplant is planned: blood group, HLA typing, infection screening, and details of potential donors.

Segunda opinião

Quando buscar uma segunda avaliação

  • You have been told you will need dialysis soon but your eGFR trend suggests a slower course.
  • Your kidneys are very large and symptomatic, and removal of a kidney before transplant is being discussed.
  • You have a family history of brain aneurysm or sudden severe headache and have not had screening.
  • Transplant is advised, but no pre-emptive transplant or living donor options were discussed.

Perguntas para fazer

  1. How fast is my kidney function falling, and what does my kidney volume say about the pace of progression?
  2. Is a medicine that slows cyst growth suitable for me?
  3. What blood pressure target should I aim for, and which medicines help most?
  4. Should my children or siblings be screened, and when?
  5. When should we plan for transplant or dialysis, and can I have a transplant before starting dialysis?
  6. Do I need screening for aneurysms or liver cysts?

Perspectiva

Prognóstico realista

PKD varies widely. Many people keep adequate kidney function into their fifties or later, while others reach kidney failure earlier. Larger kidneys, early high blood pressure and certain gene variants predict faster progression. Kidney transplant, especially from a living donor, usually gives better and longer survival than long-term dialysis, and cysts do not form in the transplanted kidney.

04 · Perguntas frequentes

Dúvidas comuns sobre polycystic kidney disease (pkd)

Will I definitely need dialysis?

Not everyone does. Many people keep adequate function for decades, though risk rises with larger kidneys, early high blood pressure and certain genes.

Can my children inherit it?

Most forms pass on with a 50 percent chance for each child. Genetic counselling and screening scans can clarify risk for relatives.

Can I have a transplant before dialysis?

Yes, a pre-emptive transplant is possible once kidney function falls low enough, and a living donor often makes this practical. Donors from the family need to be checked for the condition.

Can I fly with PKD?

Usually yes if blood pressure is controlled and kidney function is stable. Talk to your doctor if you have large painful kidneys or recent infection or bleeding into a cyst.

Clinical context

Working up polycystic kidney disease (pkd) properly

A condition like polycystic kidney disease (pkd) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a transplante de órgãos specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or transplante de órgãos specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
