Sobre peyronie's disease

O que você precisa saber

A firm lump or plaque can be felt along the shaft, and the penis curves during erection, sometimes with pain in the early phase. Erections may be weaker, and sex may be difficult. Diagnosis is by examination, with photos of the curve and sometimes ultrasound. The active phase lasts 12 to 18 months, after which the curve tends to stabilise.

Many men need only reassurance in the early phase. Medicines taken by mouth, penile traction and injection of collagenase into the plaque may help, the latter in men with stable curves. Surgery is considered once the disease is stable and the curve prevents intercourse, with plication or grafting, or an implant where erections are also poor. Treatment abroad is occasionally sought for specialist andrology care.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Description and dates of onset, any injury, pain during erection, and whether the curve has been changing or stable for at least three to six months.
  • Photographs of the erect penis from the front and side, showing the curve angle and any narrowing or hourglass deformity.
  • Penile Doppler ultrasound results, if done, showing plaque and blood flow, often with an injection to cause erection.
  • Erectile function assessment, plus your history of diabetes, high blood pressure and smoking.
  • Details of treatments tried, such as tablets, injections, traction devices or previous surgery.

Segunda opinião

Quando buscar uma segunda avaliação

  • Surgery is recommended while the curve is still changing and pain is present, which is usually the active phase.
  • An implant is advised without trying other treatments when erections are still good.
  • Erectile dysfunction and curvature are both present and only one has been addressed.
  • Treatment has failed or the curve is severe, such as an hourglass narrowing or a very large angle.

Perguntas para fazer

  1. Is my disease in the active or stable phase?
  2. How large is the curve and does it prevent intercourse?
  3. Which non-surgical options could help and what evidence supports them?
  4. If surgery, which type fits my curve, length and erections?
  5. Will my penis become shorter, and can it be protected?
  6. How will erectile function be affected?

Perspectiva

Prognóstico realista

Peyronie's disease often goes through an active phase of up to 12 to 18 months, after which the curve stabilises. Some men improve without treatment, many stay the same, and a few worsen. Injections or traction may modestly help, and surgery can straighten the penis reliably once the disease is stable. Penile implants are used when erectile dysfunction is also present. Outcome depends on curve severity, plaque calcification, erectile function and timing of treatment.

Will it go away on its own?

Some men improve slightly, but most do not fully recover without treatment. Pain often settles before the curve does.

When is surgery appropriate?

Usually when the curve has been stable for several months and is severe enough to prevent intercourse.

Will my penis get shorter?

Some loss of length is common with the disease and also can follow surgery, which is discussed beforehand.

Do I need an implant?

Only if you also have erectile dysfunction that does not respond to medicines. Otherwise, straightening surgery is usually considered first.

Clinical context

Working up peyronie's disease properly

A condition like peyronie's disease is rarely well-served by jumping straight to treatment shopping. The sequence below is what a urologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or urologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
