Pediatric VUR

Vesicoureteric reflux (VUR) is when urine flows backwards from the bladder towards the kidneys. It is often discovered after a urinary infection or an antenatal scan.

Sobre pediatric vur

O que você precisa saber

Children may have fever with urine infection, abdominal pain or wetting problems. Diagnosis uses ultrasound and a voiding cystourethrogram to grade the reflux from I to V, with a DMSA scan to look for kidney scarring. Low-grade reflux frequently settles as the child grows, but high grades with infections risk scarring.

Management includes treating infections promptly, good bladder and bowel habits, and sometimes low-dose preventive antibiotics. Surgery is considered for high-grade reflux, breakthrough infections or kidney scarring. Options are endoscopic injection of a bulking agent or ureteric reimplantation. Treatment abroad is usually considered for specialist paediatric urology review.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Voiding cystourethrogram (VCUG) or radionuclide cystogram report with the reflux grade (I to V) and which side.
  • Kidney ultrasound reports, and DMSA scan results showing any kidney scarring.
  • Urine culture results and dates of every urinary tract infection, with the antibiotics used.
  • Bladder and bowel history, including constipation, daytime wetting and toilet habits.
  • Blood pressure and kidney function blood tests.

Segunda opinião

Quando buscar uma segunda avaliação

  • Surgery or injection is advised for low-grade reflux with no infections, since many cases settle by themselves.
  • Your child has repeated breakthrough infections on antibiotic prophylaxis and no one has reviewed the bladder habits or the treatment plan.
  • A DMSA scan shows scarring and kidney function is not being followed.
  • Daytime wetting or constipation has not been treated, as these can maintain reflux and infections.

Perguntas para fazer

  1. What grade is my child's reflux and which side is affected?
  2. Is there scarring of the kidney?
  3. Is daily preventive antibiotic recommended and for how long?
  4. How are bladder and bowel habits affecting the problem?
  5. When would you recommend injection or ureteric reimplantation?
  6. How long will follow-up and blood-pressure checks continue?

Perspectiva

Prognóstico realista

Low-grade reflux commonly resolves with growth, often within a few years, and the long-term outlook is excellent when infections are prevented and treated promptly. High-grade reflux is less likely to resolve and carries a higher risk of kidney scarring. Surgery or endoscopic injection is effective in most children who need it. Outcome depends on the grade, kidney scarring, the number of infections, bladder and bowel function, and age.

04 · Perguntas frequentes

Dúvidas comuns sobre pediatric vur

Will my child grow out of reflux?

Many do, particularly with lower grades, but higher grades are less likely to resolve.

Does reflux damage the kidneys?

It can, mainly when urine infections reach the kidney, which is why prompt treatment of fevers matters.

Are antibiotics needed every day?

Sometimes, in selected children, though the benefit varies. Your specialist will advise.

Is surgery successful?

Reimplantation resolves reflux in the great majority, and endoscopic injection has a somewhat lower success rate.

Clinical context

Working up pediatric vur properly

A condition like pediatric vur is rarely well-served by jumping straight to treatment shopping. The sequence below is what a urologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or urologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
