Sobre pediatric brain tumour (medulloblastoma)

O que você precisa saber

Symptoms often come from raised pressure and blocked fluid flow, including morning headaches, vomiting, unsteady walking and clumsiness. Diagnosis is by MRI of the brain and spine, with surgery giving tissue for analysis and molecular grouping. Because it can spread through spinal fluid, staging includes a spinal MRI and fluid test.

Treatment starts with surgery to remove as much tumour as can be done safely, sometimes preceded by a shunt for fluid build-up. Radiotherapy to the brain and spine and chemotherapy follow in most children, with the dose depending on risk group and age. Rehabilitation supports recovery. Treatment abroad is often considered for proton beam therapy or specialist paediatric neuro-oncology.

Avaliação pré-viagem

Exames que devem já estar prontos

  • MRI of the whole brain and spine, with and without contrast, showing the tumour and any spread through the spinal fluid.
  • Post-operative MRI within about 48 to 72 hours, which shows the extent of tumour removed.
  • Pathology and molecular report classifying the subgroup (such as WNT, SHH, Group 3 or 4), which helps define risk.
  • Spinal fluid (CSF) cytology results, usually done about two weeks after surgery.
  • Details of any shunt or hydrocephalus treatment, hearing and kidney function tests, and endocrine baselines before chemotherapy and radiotherapy.

Segunda opinião

Quando buscar uma segunda avaliação

  • The pathology has not been classified molecularly, and risk group is therefore unclear.
  • Your child is under about three to five years old and radiotherapy is proposed or avoided without a clear discussion of alternatives.
  • The tumour has recurred or progressed and a trial or new strategy is being considered.
  • Proton therapy, which can lower dose to healthy tissue, has not been discussed.

Perguntas para fazer

  1. What subgroup and risk category is my child's tumour?
  2. What proportion of the tumour was removed, and has it spread?
  3. What treatment plan do you recommend and what is the intent?
  4. What are the long-term effects on hearing, growth, thinking and hormones?
  5. What happens if it comes back?
  6. What follow-up scans and rehabilitation will be needed?

Perspectiva

Prognóstico realista

Outlook depends on risk group. In standard-risk disease in older children treated with surgery, radiotherapy and chemotherapy, a majority are long-term survivors. High-risk disease, metastases, certain molecular subtypes and very young age carry a poorer outlook. Survivors often have late effects on thinking, hearing, growth and hormones, so rehabilitation and long-term follow-up matter. The factors that change outcome are age, extent of surgery, spread, subgroup and the radiation approach.

04 · Perguntas frequentes

Dúvidas comuns sobre pediatric brain tumour (medulloblastoma)

Is medulloblastoma curable?

Many children, particularly with standard-risk disease, are cured with combined treatment. Risk varies with age, spread and tumour biology.

Why is a biopsy alone not enough?

Surgery is both diagnostic and therapeutic here: removing as much tumour as safely possible matters, so a biopsy only is rarely the plan.

Will my child have learning problems?

Some do, especially after craniospinal radiation at a young age. Early rehabilitation and school support help.

Can proton therapy be used?

It may reduce radiation to healthy tissue and is considered in some centres. Ask your team.

Clinical context

Working up pediatric brain tumour (medulloblastoma) properly

A condition like pediatric brain tumour (medulloblastoma) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or oncologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
