Pediatric Laryngomalacia

Laryngomalacia is floppy tissue above the vocal cords that falls inward when a baby breathes in, causing noisy breathing called stridor. It is the most common cause of stridor in infants.

Sobre pediatric laryngomalacia

O que você precisa saber

The noise is usually high-pitched, starts in the first weeks, is louder when feeding, crying or lying on the back, and often peaks at six to eight months. Most babies feed and grow well, but some have reflux, feeding difficulty or pauses in breathing. Diagnosis is by flexible nasal endoscopy to watch the larynx.

Most babies need only reassurance and follow-up, as the airway matures by around 12 to 18 months. Reflux medicines are used when reflux is present. Supraglottoplasty, a small endoscopic operation that trims the floppy tissue, is for severe cases with poor weight gain, breathing difficulty or feeding trouble. Treatment abroad is seldom needed, other than for complex airways.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Description or video of the noisy breathing, noting when it is worse (feeding, crying, lying on the back) and when it began.
  • Flexible laryngoscopy report confirming laryngomalacia and its severity, and looking for a second airway problem.
  • Growth chart and feeding history, including choking, coughing, slow feeding and weight gain.
  • Reflux assessment and any medicines tried, and sleep and oxygen monitoring if breathing pauses are suspected.
  • Results of any airway endoscopy under anaesthetic, plus heart and genetic assessments if other concerns exist.

Segunda opinião

Quando buscar uma segunda avaliação

  • Surgery is recommended for a baby who is feeding and growing well, when time and observation would likely be enough.
  • Your baby has poor weight gain, breathing pauses, or blue spells and only reassurance has been given.
  • Stridor persists or is louder than expected beyond 18 to 24 months.
  • No one has examined the whole airway in a severe case to exclude other abnormalities.

Perguntas para fazer

  1. How severe is my baby's laryngomalacia, and how can we tell?
  2. Do we need to treat reflux?
  3. Is my baby gaining weight appropriately?
  4. What are the signs that mean I should seek urgent help?
  5. When would supraglottoplasty be considered?
  6. When should the noisy breathing be expected to settle?

Perspectiva

Prognóstico realista

Most infants improve without surgery, with symptoms peaking around six to eight months and settling by 12 to 24 months. About a tenth have severe disease with feeding difficulty, poor growth or breathing trouble, and supraglottoplasty is highly effective in most of them. Outcome depends on severity, associated reflux, other airway problems, neurological or genetic conditions, and the child's weight gain.

04 · Perguntas frequentes

Dúvidas comuns sobre pediatric laryngomalacia

Will my baby grow out of it?

Yes, in the large majority. Symptoms typically peak at several months and resolve by the second year.

Is the noisy breathing harmful?

In mild cases, no. It is worrying to hear, but the baby feeds and grows well. Severe cases require assessment.

Can my baby fly?

Mild cases usually can. If there are breathing difficulties, discuss with your doctor first.

Does reflux make it worse?

It often does, and treating reflux can help some babies.

Clinical context

Working up pediatric laryngomalacia properly

A condition like pediatric laryngomalacia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a otorrinolaringologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or otorrinolaringologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
