Sobre occipital neuralgia
O que você precisa saber
The pain is sharp, electric or stabbing, runs from the base of the skull towards the scalp, and the scalp may be tender to touch. Diagnosis is clinical, and relief after a local anaesthetic block of the nerve supports it. Imaging of the neck may be done to rule out other causes. Attacks may come and go over months.
Initial treatment is heat, massage, physiotherapy and anti-inflammatory medicines, with nerve-pain drugs such as gabapentin or amitriptyline. Nerve blocks with local anaesthetic and steroid often bring relief for weeks to months. For persistent cases, options include pulsed radiofrequency, occipital nerve stimulation and surgery to free the nerve. Treatment abroad is rarely required.
Avaliação pré-viagem
Exames que devem já estar prontos
- A description of the pain: shooting or electric quality, side, triggers, and tenderness over the nerve at the back of the head.
- MRI of the brain and upper cervical spine, which is usually requested to exclude other causes.
- A headache diary noting frequency, duration and any other symptoms such as nausea or light sensitivity.
- Records of previous nerve blocks or injections, including how long relief lasted.
- A list of medicines tried, such as nerve-pain drugs, with doses and results.
Segunda opinião
Quando buscar uma segunda avaliação
- The diagnosis is based on symptoms alone and migraine or cervicogenic headache has not been excluded.
- A nerve block gives only brief relief and a destructive or implanted treatment is proposed.
- You have had a neck injury or surgery and the pain started afterwards.
- Your pain has red-flag features, such as new severe headache, weakness or visual changes.

Perguntas para fazer
- Is this really occipital neuralgia, or could it be migraine or a neck problem?
- What did my scans show about the upper neck?
- What level of relief can a nerve block give, and how often can it be repeated?
- Which medicines suit me, and what are their side effects?
- What do botulinum toxin, pulsed radiofrequency or nerve stimulation add?
- What can I do to protect my neck and avoid triggers?
Perspectiva
Prognóstico realista
Many people with occipital neuralgia improve with nerve blocks, heat, physiotherapy and medicines, and some have long periods of relief. In others the pain is persistent or recurs, and further procedures may be considered. Outcome depends on the cause, such as a compressed nerve or a neck injury, and on getting the diagnosis right.

Is occipital neuralgia the same as migraine?
No, although they can coexist and be confused. Occipital neuralgia causes brief shooting pain at the back of the head with tender nerves, while migraine involves throbbing pain with other features.
How does a nerve block work?
A local anaesthetic, usually with a steroid, is injected near the occipital nerve. It can give relief lasting weeks, and it also helps confirm the diagnosis.
Will it go away on its own?
Some cases settle, especially after a neck strain or irritation. Others become chronic and need continued treatment.
Can neck posture cause it?
Yes. Tight neck muscles, arthritis of the upper neck and prolonged poor posture can irritate the nerves. Physiotherapy and posture changes often help.
Clinical context
Working up occipital neuralgia properly
A condition like occipital neuralgia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a neurologia e neurocirurgia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or neurologia e neurocirurgia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
