Insuficiência mitral

Mitral regurgitation is leakage of blood backwards through the heart's mitral valve each time the heart beats. It can come on suddenly or develop slowly over many years.

Sobre insuficiência mitral

O que você precisa saber

Mild leakage causes no symptoms. Severe leakage leads to breathlessness, fatigue, palpitations and ankle swelling, and can enlarge the heart and cause atrial fibrillation. Diagnosis is by listening for a murmur and by echocardiography, which shows the cause and severity. Careful monitoring is important, since delaying treatment too long can permanently weaken the heart.

Mild cases are watched with regular echocardiograms. Medicines can ease symptoms but do not fix the valve. Severe leakage is treated with surgery, where repair of the patient's own valve is preferred when possible, and replacement with a mechanical or tissue valve if not. For selected higher-risk patients, a catheter-based edge-to-edge clip repair is an option. Patients travel abroad for specialist valve repair.

Avaliação pré-viagem

Exames que devem já estar prontos

  • An echocardiogram report, and ideally the images, showing the cause, severity, valve structure and heart size and pumping strength.
  • A transoesophageal echocardiogram, which is usually requested to plan repair.
  • Coronary angiography or CT angiography, especially if you are over about 40 or have risk factors.
  • An ECG and any record of atrial fibrillation or heart failure symptoms.
  • Blood tests including kidney function and clotting, and a list of medicines such as blood thinners.

Segunda opinião

Quando buscar uma segunda avaliação

  • Surgery is advised but you have no symptoms and want to confirm the timing.
  • You are told the valve must be replaced and wonder whether repair is possible.
  • You are high risk for open surgery and a catheter-based option is being discussed.
  • The leak is described as secondary to a weak heart muscle and treatment options differ between centres.

Perguntas para fazer

  1. Is my leak caused by the valve itself or by the heart muscle, and how severe is it?
  2. Is my valve suitable for repair rather than replacement?
  3. Is it safe to wait, or is delay damaging my heart?
  4. If replaced, would a mechanical or tissue valve suit me, and what does each mean for blood thinners?
  5. Am I a candidate for a catheter edge-to-edge procedure?
  6. What follow-up scans will I need afterwards?

Perspectiva

Prognóstico realista

Outlook is generally good when severe leakage from a degenerative valve is repaired before the heart weakens. Repair usually gives better long-term results than replacement where it is possible. Waiting until the heart enlarges or pumps poorly worsens results. Mechanical valves need lifelong warfarin; tissue valves wear out over time. Outcome also depends on age, heart function and other illnesses.

04 · Perguntas frequentes

Dúvidas comuns sobre insuficiência mitral

Does mitral regurgitation always need surgery?

No. Mild or moderate leaks are monitored with echocardiograms. Surgery is advised for severe leaks that cause symptoms, heart enlargement or falling pumping strength, ideally before the heart is damaged.

Is repair better than replacement?

When the valve can be repaired, it usually gives better survival and heart function and may avoid lifelong blood thinners. Not every valve is repairable, and a surgeon experienced in repair makes the difference.

Can I fly with a leaking mitral valve?

If you have no significant symptoms, flying is usually fine. If you have breathlessness, fluid retention or irregular rhythm, you should have these controlled first and discuss travel with your cardiologist.

How long is recovery after valve surgery?

Hospital stay is typically about a week, and full recovery after open-heart surgery takes about six to twelve weeks. Catheter-based repair usually has a shorter recovery.

Clinical context

Working up insuficiência mitral properly

A condition like insuficiência mitral is rarely well-served by jumping straight to treatment shopping. The sequence below is what a cirurgia cardíaca specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or cirurgia cardíaca specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
