Lobular Breast Cancer

Lobular breast cancer is the second most common type of breast cancer, starting in the milk-producing lobules. It tends to grow in a diffuse pattern and is usually hormone receptor positive.

Sobre lobular breast cancer

O que você precisa saber

It may be felt as a thickening or fullness rather than a distinct lump, and it can be hard to see on mammograms. Diagnosis involves breast imaging, often with MRI, and a core needle biopsy that identifies the lobular type and receptor status. It often grows slowly but may spread to unusual sites. It can affect both breasts.

Treatment is usually surgery, either breast conserving or mastectomy, with sentinel lymph node biopsy to check the armpit. Because lobular cancer can be widespread, mastectomy is more often needed. Radiotherapy may follow. Hormone therapy is central as tumours are usually oestrogen sensitive, and chemotherapy is used selectively according to risk. Treatment abroad is sometimes considered for surgery and reconstruction.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Core biopsy report confirming lobular type, grade, ER, PR and HER2 status.
  • Breast MRI report, which is often advised because lobular cancer can be larger or multifocal than mammography suggests.
  • Mammogram and ultrasound reports, and axillary ultrasound or nodal findings.
  • Staging scans if the cancer is advanced, and a family history of breast or ovarian cancer, with BRCA or CDH1 results if tested.
  • Any genomic test result, such as Oncotype DX, which can guide the decision about chemotherapy.

Segunda opinião

Quando buscar uma segunda avaliação

  • Breast-conserving surgery has been advised and you want to confirm with MRI that the cancer is not more extensive.
  • Mastectomy is recommended and you want to know if conserving the breast is possible.
  • Chemotherapy is advised and you want to see whether genomic testing could avoid it.
  • You have a family history of diffuse gastric cancer or CDH1 variants, which need genetic counselling.

Perguntas para fazer

  1. Is the cancer hormone receptor-positive, and what is its HER2 status?
  2. Has MRI shown the full extent, and is the cancer in more than one area?
  3. Am I suitable for breast-conserving surgery or is mastectomy safer?
  4. Will I need chemotherapy, radiotherapy or hormone therapy, and for how long?
  5. Should I have genetic testing?
  6. What follow-up imaging is best for lobular cancer, given it can be hard to see on mammograms?

Perspectiva

Prognóstico realista

Lobular cancer generally has a good outlook when found at an early stage, as it is usually hormone receptor-positive and responds well to endocrine therapy. Outcomes depend on size, lymph node involvement and grade. Late recurrences can occur many years after treatment, so hormone therapy and follow-up often continue for five to ten years. Lobular cancer can spread to unusual sites, such as the peritoneum.

04 · Perguntas frequentes

Dúvidas comuns sobre lobular breast cancer

Why is lobular cancer harder to find?

It grows in thin lines of cells rather than a solid lump, so it can be less visible on mammograms. MRI or ultrasound is often used to define its extent.

Is it more likely to spread to both breasts?

Lobular cancer is somewhat more likely to be multifocal or to occur in both breasts. Imaging before surgery helps to detect this.

Why is hormone therapy so important?

Most lobular cancers are driven by oestrogen. Endocrine therapy for five to ten years lowers the risk of recurrence, including late recurrence.

Clinical context

Working up lobular breast cancer properly

A condition like lobular breast cancer is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or oncologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
