Gastrointestinal Stromal Tumour (GIST)

A gastrointestinal stromal tumour (GIST) is a rare sarcoma arising from cells in the wall of the digestive tract, most often the stomach or small bowel. It is typically diagnosed in people over 50.

Sobre gastrointestinal stromal tumour (gist)

O que você precisa saber

Small tumours may cause no symptoms and are found incidentally. Larger ones cause bleeding, anaemia, abdominal pain, a mass or bowel obstruction. Diagnosis uses CT or MRI, endoscopic ultrasound with biopsy, and immunostaining for KIT (CD117) or DOG1, with mutation testing to guide treatment. Risk of recurrence depends on tumour size, site and mitotic rate.

Complete surgical removal is the main treatment for localised GIST. Targeted drugs such as imatinib, a tyrosine kinase inhibitor, are used after surgery for higher-risk tumours, before surgery to shrink large ones, and for advanced disease, usually taken long-term. Regular imaging follows treatment. Because GIST is rare, patients often seek care at specialist sarcoma centres, including abroad.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Pathology report with tumour site, size, mitotic count and the mutation analysis for KIT and PDGFRA.
  • CT or MRI scans of abdomen and pelvis, and endoscopic ultrasound where the tumour is in the stomach.
  • Operation notes and whether the tumour capsule was intact or ruptured, if surgery has already been done.
  • A record of any imatinib or other targeted therapy, with dose, dates, side effects and response scans.
  • Blood results including blood count and liver function, plus heart assessment if relevant.

Segunda opinião

Quando buscar uma segunda avaliação

  • Mutation testing has not been done, or the result is unclear, as it decides which drug and dose work.
  • The tumour is large or in a difficult location and you are told it cannot be removed.
  • The tumour is growing on imatinib and next-line drugs are being considered.
  • You are unsure whether to take adjuvant imatinib after surgery and for how long.

Perguntas para fazer

  1. What are the size, mitotic rate and risk category of my tumour?
  2. Which mutation does it carry, and does it respond to imatinib?
  3. Is surgery possible now, or should drug treatment shrink it first?
  4. Do I need adjuvant imatinib after surgery, and for how many years?
  5. How will side effects and response be monitored?
  6. What scans and follow-up will I need?

Perspectiva

Prognóstico realista

Outlook depends on tumour size, site, mitotic rate and whether it was removed intact. Small low-risk tumours are often cured by surgery, while higher-risk tumours may recur and benefit from imatinib after surgery. Advanced GIST is rarely cured, but imatinib controls disease for years in many patients, with later-line drugs available if it stops working. Treatment is long term and requires regular scans and blood tests.

04 · Perguntas frequentes

Dúvidas comuns sobre gastrointestinal stromal tumour (gist)

Is GIST a type of cancer?

Yes, it is a sarcoma of the digestive tract wall. Its behaviour ranges from low to high risk depending on size, site and mitotic rate.

How long do I take imatinib?

For higher-risk tumours after surgery, imatinib is typically given for three years or longer. In advanced disease, it is continued as long as it works and is tolerated.

Will chemotherapy help?

Standard chemotherapy is not effective for GIST. Targeted drugs like imatinib are the key treatment.

Clinical context

Working up gastrointestinal stromal tumour (gist) properly

A condition like gastrointestinal stromal tumour (gist) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or oncologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
