Female Pattern Hair Loss

Female pattern hair loss is a gradual, genetically influenced thinning of the hair over the crown and central parting. It becomes more common after the menopause but can begin in the teens or twenties.

Sobre female pattern hair loss

O que você precisa saber

Hair becomes finer and the parting widens, while the frontal hairline is usually preserved and complete baldness is rare. Diagnosis rests on the pattern of thinning and examination of the scalp, often with blood tests for iron, thyroid function and hormones to exclude other causes. The condition progresses slowly over years and tends to continue without treatment.

Topical minoxidil is the usual first treatment, sometimes with anti-androgen medicines, iron correction where needed, and low-level light therapy. Camouflage products and hairpieces help some women. Hair transplantation, such as sapphire FUE, may be considered when thinning is stable, donor hair at the back of the scalp is good and medicines have not given enough density. Some people travel abroad for transplant surgery.

Avaliação pré-viagem

Exames que devem já estar prontos

  • A dermatology note confirming female pattern (androgenetic) hair loss, with the pattern graded, for example on the Ludwig scale.
  • Blood tests usually requested to exclude other causes of shedding: ferritin, thyroid function, vitamin D and, where relevant, hormone profile.
  • Dated scalp photographs or trichoscopy images showing how the thinning has progressed over time.
  • A list of treatments tried so far, such as minoxidil or anti-androgen tablets, with doses, duration and response.
  • A note of your donor-area density, since a transplant needs enough stable hair at the back and sides of the scalp.

Segunda opinião

Quando buscar uma segunda avaliação

  • Your diagnosis has not been confirmed by examination or trichoscopy, and the thinning came on suddenly or in patches rather than gradually.
  • A transplant has been recommended but your donor hair is sparse or the thinning is still spreading quickly.
  • You are pregnant, breastfeeding or planning pregnancy and have been offered medicines for hair loss.
  • Treatment has been advised without blood tests for iron, thyroid or hormonal causes.

Perguntas para fazer

  1. Is this definitely female pattern hair loss, or could iron deficiency, thyroid disease or another cause be contributing?
  2. How far has it progressed, and is my donor area suitable for a transplant?
  3. Which medical treatments should I try first, and how long before we can judge whether they work?
  4. If I have a transplant, will I still need medicine to protect the hair I already have?
  5. What happens to my hair over the next few years if I do nothing?
  6. How will you plan the hairline and density, and how many sessions might I need?

Perspectiva

Prognóstico realista

Female pattern hair loss is a long-term, slowly progressive condition and does not resolve by itself. Treatment usually aims to slow the thinning and thicken existing hair rather than cure it. Many women see stabilisation with consistent medical treatment. A transplant can add density where donor hair is adequate, but results depend on donor supply, ongoing thinning of untreated hair, and results take around 9 to 12 months to appear.

Will medicine alone be enough?

Many women see thinning slow or partly reverse with consistent treatment such as minoxidil, but results build over six to twelve months and fade if treatment stops. A transplant is considered when medicine has plateaued and donor hair is adequate.

Can women have a hair transplant?

Yes, if the thinning is stable and there is enough dense donor hair at the back and sides. Women with diffuse thinning across the whole scalp are often poorer candidates, so assessment first matters.

When will I see results from a transplant?

Transplanted hairs shed within weeks, then regrow from about three to four months, with final density usually judged at 9 to 12 months. Continued medical treatment protects existing hair.

Clinical context

Working up female pattern hair loss properly

A condition like female pattern hair loss is rarely well-served by jumping straight to treatment shopping. The sequence below is what a cirurgia estética specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or cirurgia estética specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
