Excess Eyelid Skin

Excess eyelid skin, known as dermatochalasis, is loose skin and sometimes fat in the upper or lower eyelids. It gives a tired look and can, in some people, hang low enough to block the upper field of vision.

Sobre excess eyelid skin

O que você precisa saber

It develops gradually with ageing, and may run in families. People notice heavy lids, puffiness, difficulty with make-up and, if severe, a restricted view above. A surgeon examines the lids and brow position, tests the visual field when the upper lid hangs low, and takes photographs. It does not harm the eye, but does not improve by itself.

Blepharoplasty removes excess skin and fat through a fine incision hidden in the lid crease (upper) or just below the lashes or inside the lid (lower). It is usually done under local anaesthetic with sedation as a day case, and bruising settles in one to two weeks. A brow lift may be needed if a low brow contributes. Travelling abroad for eyelid surgery should include time for suture removal.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Clear front and side photographs of your eyes, which surgeons use to plan skin removal and for any insurance purposes.
  • Eyelid and eye examination, including brow position, to see whether a brow lift is also needed.
  • A visual field test if the upper lid droops enough to affect sight.
  • A history of dry eyes, thyroid eye disease, previous eyelid surgery, or glaucoma.
  • A list of medicines, especially blood thinners and aspirin, and any history of poor wound healing.

Segunda opinião

Quando buscar uma segunda avaliação

  • Your upper lids appear heavy because the eyebrows are low, and a brow lift has not been discussed.
  • You have dry eyes or thyroid eye disease and have been told you can have surgery without a specific assessment.
  • You were told that lower lid surgery will remove puffiness but have lax lids, risking the lid pulling down.
  • Two surgeons describe very different amounts of skin removal.

Perguntas para fazer

  1. Is my problem skin, fat, drooping brows, or a combination?
  2. Do I need upper lids, lower lids, or both?
  3. How will my dry eye symptoms or other eye conditions affect the surgery?
  4. What are the risks, such as asymmetry, lid retraction and difficulty closing the eyes?
  5. How long will swelling and bruising last, and when can I fly home?
  6. How long will the result last?

Perspectiva

Prognóstico realista

Blepharoplasty generally gives a natural, long-lasting rejuvenation of the eye area, with results often lasting many years. Swelling and bruising settle over two to three weeks, and final shape appears after a few months. Ageing continues, so some change over time is normal. Outcome depends on skin quality, brow position, and eye health. Complications such as dry eye, asymmetry or lid retraction are uncommon but can occur.

04 · Perguntas frequentes

Dúvidas comuns sobre excess eyelid skin

Is blepharoplasty cosmetic or medical?

Either. If drooping skin blocks your upper field of vision it may be functional, whereas lower lid surgery is mostly cosmetic.

Will there be visible scars?

Upper lid scars lie in the natural crease, and lower lid cuts are usually just beneath the lashes or inside the lid. They typically fade well.

How long does recovery take?

Bruising and swelling usually settle in about two weeks, and stitches come out in about a week. Final results appear over some months.

Clinical context

Working up excess eyelid skin properly

A condition like excess eyelid skin is rarely well-served by jumping straight to treatment shopping. The sequence below is what a cirurgia estética specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or cirurgia estética specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
