Conductive Hearing Loss

Conductive hearing loss occurs when sound does not pass properly through the outer or middle ear. Causes include wax, fluid, a perforated eardrum and damage to the tiny hearing bones.

Sobre conductive hearing loss

O que você precisa saber

Sounds seem muffled or quiet, though speech is usually clear when loud enough, and people may feel their own voice is loud. An ear examination and an audiogram show that the problem is in sound conduction rather than the inner ear. Many causes, such as wax or fluid, are reversible.

Treatment depends on the cause: wax removal, treating infection and grommets for persistent fluid. Eardrum repair or ossiculoplasty, rebuilding the hearing bones, can restore hearing in suitable cases. When surgery is not possible, hearing aids or a bone-anchored hearing aid (BAHA) that sends sound through the skull bone are options. Treatment abroad is considered for complex reconstructive ear surgery.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Pure tone audiogram with bone and air conduction, showing the air-bone gap, and tympanometry.
  • ENT examination findings or microscope images of the eardrum and middle ear.
  • High-resolution CT scan of the temporal bones if bone damage, otosclerosis or a malformation is suspected.
  • Records of previous ear surgery, infections and head injuries, and family history of hearing loss.
  • A note of whether one or both ears are affected, any ear discharge, and past use of hearing aids.

Segunda opinião

Quando buscar uma segunda avaliação

  • You have been told nothing can be done, but your audiogram shows a pure conductive loss that is often correctable.
  • You are offered a hearing aid only, and have not discussed ossicular surgery or a bone-anchored implant.
  • Hearing worsened after ear surgery or an infection, and you want to know the cause.

Conversando com seu médico

Perguntas para fazer

  1. What is causing my conductive loss: fluid, eardrum, ear bone damage or otosclerosis?
  2. Could surgery restore hearing, and how large an improvement is realistic?
  3. When would a bone-anchored hearing device be better than surgery?
  4. What are the risks to my hearing, balance and taste after the operation?
  5. How long until I can fly or swim after surgery?

Perspectiva

Prognóstico realista

Outlook is often good, because many conductive losses can be corrected. Ossicular reconstruction improves hearing to a useful level in a majority of patients, though results vary with the extent of ear damage and the prosthesis used. A bone-anchored device can bypass the middle ear when surgery is unsuitable. Long-term outcomes depend on the cause and on the health of the Eustachian tube.

Can conductive hearing loss be reversed?

Often yes, if the cause is wax, fluid, a perforation or broken ear bones, which can be treated medically or surgically. Results depend on the cause and the condition of the ear.

Do I need a hearing aid or surgery?

It depends on the cause. Surgery can restore hearing in many cases, while hearing aids or bone-anchored devices help when surgery is unsuitable or declined.

Can I fly with it?

Yes. If you have fluid or Eustachian tube problems, pressure changes may be uncomfortable; chewing gum or a decongestant may help if your doctor agrees.

Clinical context

Working up conductive hearing loss properly

A condition like conductive hearing loss is rarely well-served by jumping straight to treatment shopping. The sequence below is what a otorrinolaringologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or otorrinolaringologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
