Subdural Hematoma (Chronic)

A chronic subdural haematoma is a slowly growing collection of old blood between the brain and its outer covering. It usually follows a minor head knock, often forgotten, in an older person.

Sobre subdural hematoma (chronic)

O que você precisa saber

Symptoms develop over weeks and include headache, confusion, drowsiness, unsteadiness, weakness on one side and sometimes seizures. A CT or MRI scan of the head makes the diagnosis. Risk is higher with blood-thinning drugs, alcohol misuse, falls and brain shrinkage with age. It can worsen quickly, so it needs prompt assessment.

Small collections without significant symptoms may be watched with repeat scans, and blood-thinning medicines are reviewed. When symptoms are present, the standard treatment is drainage through one or two small holes in the skull, called burr-hole drainage, which often brings quick improvement. Recurrence occurs in a minority and may need another drain or a procedure to block the feeding vessel. It is an emergency, so treatment is usually local.

Avaliação pré-viagem

Exames que devem já estar prontos

  • CT or MRI of the head showing the size and side of the collection and any shift of the brain.
  • A list of blood thinners and antiplatelet medicines, with the date of the last dose, as these affect timing of surgery.
  • Clotting tests, platelet count and kidney function results.
  • Notes on the head injury or fall, symptom timeline, and any seizures, confusion, weakness or headache.
  • Previous brain imaging for comparison, and records of other conditions such as alcohol use, liver disease or shunts.

Segunda opinião

Quando buscar uma segunda avaliação

  • You have worsening headache, confusion, weakness or drowsiness and need urgent specialist assessment; do not delay care for travel.
  • You were advised just to observe the collection, but your symptoms or scan are getting worse.
  • The haematoma has returned after earlier drainage.

Conversando com seu médico

Perguntas para fazer

  1. Is the collection large enough or causing enough pressure to need drainage now?
  2. Which blood thinners must be paused, and who should decide that?
  3. Is burr-hole drainage planned, and could embolisation of the middle meningeal artery reduce recurrence?
  4. What is the chance it will return, and what symptoms should prompt urgent review?
  5. When is it safe to fly after surgery or after the scan shows resolution?

Perspectiva

Prognóstico realista

Drainage through small burr holes usually brings quick improvement in headache, weakness and confusion, and most patients recover well. Recurrence occurs in a minority and is more likely with blood-thinning medicines, large collections and older age. Small, symptom-free collections may be watched with repeat scans. Delay in treating a large symptomatic collection can lead to serious harm.

Does a chronic subdural haematoma always need surgery?

No. Small collections without much pressure or symptoms may be watched with repeat scans. Surgery is usually advised when there are significant symptoms such as weakness, confusion or drowsiness.

Can it come back after draining?

Yes, in a minority of cases, especially with blood thinners or large collections. A drain left in for a day or two and, in some centres, middle meningeal artery embolisation reduce the risk.

Can I fly after treatment?

Your surgeon will advise, usually after the repeat scan shows the collection has settled and you are well, often several weeks later. Do not travel with worsening headache or confusion.

Clinical context

Working up subdural hematoma (chronic) properly

A condition like subdural hematoma (chronic) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a neurologia e neurocirurgia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or neurologia e neurocirurgia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
