Cerebral AVM

A cerebral arteriovenous malformation (AVM) is a tangle of abnormal blood vessels in the brain, usually present from birth. It can bleed or cause seizures.

Sobre cerebral avm

O que você precisa saber

Many AVMs are found after a seizure, a haemorrhage or persistent headaches, and some by chance. Diagnosis uses MRI and catheter angiography, which show the feeding vessels and drainage. Specialists grade the AVM, for example with the Spetzler-Martin scale, to weigh the risk of treatment against the yearly risk of bleeding. An AVM that has already bled has a higher chance of bleeding again.

Management options are surgical resection, embolisation through a catheter, stereotactic radiosurgery such as Gamma Knife, or observation, often in combination. Surgery gives immediate removal and suits accessible AVMs. Radiosurgery closes small deep ones over a couple of years. Seizures are controlled with medicines. Unruptured AVMs sometimes call for careful discussion of whether treatment risks outweigh benefits.

Avaliação pré-viagem

Exames que devem já estar prontos

  • MRI and catheter angiography reports with images, showing nidus size, location, feeding arteries and draining veins.
  • A Spetzler-Martin grade or similar classification and the pattern of venous drainage.
  • A history of haemorrhage, seizures or neurological deficits and the medicines used.
  • Details of any prior embolisation or radiosurgery, with doses and dates.
  • Family history and, if young, any signs of hereditary vascular conditions.

Segunda opinião

Quando buscar uma segunda avaliação

  • Treatment has been advised for an unruptured AVM that you have no symptoms from.
  • Observation has been advised but the AVM has bled before.
  • Your AVM is in a deep or eloquent area and has been called untreatable.
  • A combined approach has been proposed and you wish to compare it with radiosurgery alone.

Perguntas para fazer

  1. What are the size, location and grade of my AVM?
  2. What is my yearly bleeding risk?
  3. Is treatment safer than observation for me?
  4. Which option is best: surgery, embolisation, radiosurgery or a combination?
  5. How long until I know if radiosurgery has worked?
  6. How will seizures be managed?

Perspectiva

Prognóstico realista

Outlook varies with the AVM's size, location and bleeding history. Untreated AVMs carry a yearly risk of bleeding that is higher after a previous bleed. Surgical removal of a small, accessible AVM usually cures it. Radiosurgery obliterates many small AVMs but takes two to three years to work, during which bleeding risk remains. Embolisation is usually an aid to other treatments. Treatment itself carries risks of neurological deficit, which is why selection is individualised.

04 · Perguntas frequentes

Dúvidas comuns sobre cerebral avm

Does every AVM need treatment?

No. Some unruptured AVMs are monitored if treatment risk exceeds bleeding risk.

How soon does radiosurgery work?

Closure takes about two to three years, confirmed by repeat imaging.

Can the AVM recur?

After confirmed complete removal, recurrence is uncommon, especially in adults but can happen in children.

Can I fly with an AVM?

Generally yes if stable; discuss seizures and symptoms with your doctor.

Clinical context

Working up cerebral avm properly

A condition like cerebral avm is rarely well-served by jumping straight to treatment shopping. The sequence below is what a neurologia e neurocirurgia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or neurologia e neurocirurgia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
