Câncer de bexiga
Bladder cancer starts in the lining of the bladder. It is most often found in older adults, particularly smokers and people with certain workplace chemical exposures.

Sobre câncer de bexiga
O que você precisa saber
The most common sign is blood in the urine without pain. Other symptoms include frequent or painful urination. Diagnosis involves cystoscopy, a look inside the bladder with a thin camera, and urine tests and CT imaging. Removed tissue is examined to find out whether the tumour has grown into the bladder muscle, which largely decides the outlook and treatment. Superficial tumours often return and need long-term checks.
Most tumours are limited to the lining and are removed through the urethra by transurethral resection (TURBT), sometimes followed by bladder instillations of chemotherapy or BCG to lower recurrence. Tumours that invade muscle need radical cystectomy, removal of the bladder, or chemoradiotherapy, usually with chemotherapy first. Surveillance cystoscopy continues for years. Patients sometimes travel abroad for complex surgery or bladder reconstruction.
Avaliação pré-viagem
Exames que devem já estar prontos
- The cystoscopy report and the pathology from the transurethral resection, stating tumour grade, stage and whether bladder muscle was present in the specimen.
- CT urogram or MRI reports of the kidneys, ureters and pelvis, plus chest imaging when invasion is suspected.
- Urine cytology results and blood tests including kidney function and haemoglobin.
- Any previous bladder tumour history, BCG or chemotherapy treatments received, with dates.
- Details of smoking history and occupational exposure to dyes or chemicals, which influence risk and follow-up.
Segunda opinião
Quando buscar uma segunda avaliação
- Removal of the bladder has been advised and you want to confirm that the tumour has truly invaded muscle.
- The first resection did not include muscle layer or was incomplete, and a repeat resection has not been discussed.
- A high-grade non-invasive tumour keeps recurring despite BCG treatment.
- You are choosing between bladder removal and chemoradiotherapy for bladder preservation, or between types of urinary diversion.

Perguntas para fazer
- What are the grade and stage of my tumour, and has it reached the muscle?
- Do I need a repeat resection or further imaging before deciding treatment?
- Is chemotherapy before surgery recommended for me?
- What are the options for preserving the bladder and what are their limits?
- If the bladder is removed, what are my urinary diversion options and how will they affect daily life?
- What follow-up schedule will I need?
Perspectiva
Prognóstico realista
The outlook depends mainly on whether the cancer has invaded the bladder muscle. Non-muscle-invasive tumours are often controlled with local treatment but frequently recur, so many people need years of cystoscopic surveillance, and a minority progress to invasive disease. Muscle-invasive cancer needs radical treatment such as cystectomy or chemoradiotherapy and, if caught before spreading, a substantial proportion are cured. Spread to lymph nodes or distant organs lowers cure rates considerably. Stopping smoking improves results.

What is the difference between invasive and non-invasive bladder cancer?
Non-invasive cancer stays in the lining and is treated through the urethra. Invasive cancer has grown into the muscle wall and usually needs major treatment such as bladder removal or chemoradiotherapy.
Can bladder cancer come back?
Yes, particularly the non-invasive type, which often recurs and needs regular cystoscopy. Muscle-invasive disease can also recur or spread, so follow-up scans continue after treatment.
Do I have to lose my bladder?
Not always. Early tumours are removed endoscopically, and selected invasive tumours can be treated with chemoradiotherapy. Bladder removal is the standard for many muscle-invasive cancers.
Can I fly after a bladder tumour resection?
Usually after the bleeding settles and your surgeon clears you, typically some days to weeks after. Blood in the urine or a catheter should be discussed before flying.
Clinical context
Working up câncer de bexiga properly
A condition like câncer de bexiga is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or oncologia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
