Asthenozoospermia

Asthenozoospermia is reduced sperm motility in a semen sample, meaning too few sperm swim forwards well. It lowers the chance of natural conception and is one of the causes of male-factor infertility.

Sobre asthenozoospermia

O que você precisa saber

There are usually no symptoms; it is found on semen analysis after a year of trying to conceive without success, and the test is repeated to confirm it. Causes include varicocele, infection, heat, smoking, obesity, certain medicines and genetic factors, and often none is found. Assessment includes history, examination, hormone tests and sometimes scrotal ultrasound.

Treatment starts with lifestyle changes, treating infection or varicocele, and stopping smoking, and sometimes antioxidants or hormones. If motility is mildly reduced, intrauterine insemination may be tried. For more severe reduction, IVF with intracytoplasmic sperm injection (ICSI), which places one sperm into each egg, gives the best chance. Couples may travel abroad for fertility treatment.

Avaliação pré-viagem

Exames que devem já estar prontos

  • Two or more semen analyses, taken at least a few weeks apart, with motility, count and morphology reported to WHO standards.
  • Hormone tests (FSH, LH, testosterone) and scrotal ultrasound for varicocele, if done.
  • Sperm DNA fragmentation test result, where it has been requested.
  • History of infections, fever, medicines, smoking, heat exposure and previous surgery.
  • Female partner's fertility tests: ovarian reserve, tubal assessment and age.

Segunda opinião

Quando buscar uma segunda avaliação

  • Only one semen analysis has been done and ICSI is being recommended.
  • A varicocele or infection has not been examined, and may be treatable.
  • ICSI has failed repeatedly and you want to explore sperm selection or DNA fragmentation.
  • Motility is zero or very low, and the question is whether the sperm are alive or genetically abnormal.

Conversando com seu médico

Perguntas para fazer

  1. Is my low motility persistent, and what could be causing it?
  2. Is there a treatable cause such as varicocele, infection or lifestyle factors?
  3. At what point would you recommend ICSI rather than IUI?
  4. Should I have genetic or DNA fragmentation testing?
  5. How do my partner's age and ovarian reserve affect the plan?
  6. What are the success rates in couples like us?

Perspectiva

Prognóstico realista

Reduced motility does not mean infertility, and many couples conceive naturally or with assisted conception. ICSI selects a single sperm for each egg and bypasses motility problems, giving fertilisation rates close to those of other causes. The chance of a live birth depends largely on the female partner's age and egg reserve, as well as the underlying sperm problem.

04 · Perguntas frequentes

Dúvidas comuns sobre asthenozoospermia

Can motility improve naturally?

It can. Stopping smoking, reducing heat exposure, treating infection or varicocele, and managing weight may help, and sperm quality varies from sample to sample.

Do I need ICSI?

Not always. Mild cases may conceive naturally or with IUI, while severe motility problems usually need ICSI.

Will my child inherit the problem?

Most causes are not inherited, but a few genetic conditions can affect sperm movement. Genetic counselling is offered in severe or unexplained cases.

Clinical context

Working up asthenozoospermia properly

A condition like asthenozoospermia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilidade e fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilidade e fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
