Achilles Tendon Rupture
An Achilles tendon rupture is a complete or near-complete tear of the large tendon at the back of the ankle. It usually happens during sudden push-off in sport, most often in active adults aged 30 to 50.

Sobre achilles tendon rupture
O que você precisa saber
Many people feel or hear a pop at the back of the heel, as if they were kicked, followed by pain, swelling and difficulty rising onto the toes. Doctors find a gap in the tendon and a positive Thompson calf-squeeze test, and ultrasound or MRI confirms the tear. Without treatment the tendon heals long and weak, so push-off strength is reduced.
Treatment is either a structured non-operative programme, with the foot held in a boot that is gradually lowered while loading begins, or surgical repair of the tendon. Both give good results when rehabilitation is followed, and surgery carries a slightly lower re-rupture risk but a risk of wound problems. Return to sport usually takes around six months or longer.
Avaliação pré-viagem
Exames que devem já estar prontos
- Ultrasound or MRI report showing the site and size of the tear and the gap between the tendon ends.
- Date and mechanism of injury, because timing affects whether repair is acute or delayed reconstruction.
- Record of the Thompson (calf squeeze) test and any splint or equinus boot used since injury.
- Medication history, particularly recent fluoroquinolone antibiotics or steroid injections near the tendon, which are linked to ruptures.
- Notes on diabetes, smoking and wound-healing history, which influence the risk of skin problems after surgery.
Segunda opinião
Quando buscar uma segunda avaliação
- You are told surgery is the only option, although functional non-operative treatment is accepted for many acute ruptures.
- The injury is several weeks or months old and a gap has formed, which may need reconstruction rather than simple repair.
- You have had a rupture on the other side or a previous repair that failed.
- You are an athlete or manual worker and wish to compare re-rupture risk and return-to-sport timelines between options.

Perguntas para fazer
- Is my rupture in the midportion or at the heel attachment, and does that change the repair?
- What are the re-rupture and wound-complication risks of surgery compared with non-operative care in my case?
- How long will I be in a boot, and when can I put weight on the foot?
- When can I realistically return to driving, work and sport?
- Will I regain full calf strength, and what rehabilitation do I need?
- What symptoms of re-rupture or blood clot should I watch for while travelling home?
Perspectiva
Prognóstico realista
Most people regain good walking function after an Achilles rupture whether treated by repair or by accelerated non-operative care. Full recovery of calf strength and return to sport usually takes around six to twelve months, and some residual calf weakness is common. Re-rupture risk is low with modern early-motion protocols. Timing of treatment, age, smoking and diabetes affect results.

Do I always need surgery?
No. Many acute ruptures heal well with a structured boot and early-movement programme, and surgery lowers re-rupture risk slightly but adds wound risks. The choice depends on age, activity and timing.
How long until I can walk normally?
Most people walk in a boot within weeks and without it by around two to three months. Running and sport usually take six months or more.
Can I fly after a rupture or repair?
Yes, but long flights raise the risk of leg blood clots while the leg is immobilised. Your doctor may advise movement, compression or preventive medication.
Clinical context
Working up achilles tendon rupture properly
A condition like achilles tendon rupture is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ortopedia specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or ortopedia specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
