Blood is drawn by apheresis to collect T-cells, which are sent to a laboratory and modified to carry a chimeric antigen receptor targeting a marker on the cancer cells. While they are manufactured, which can take a few weeks, you may receive bridging therapy. Shortly before the infusion, a short course of lymphodepleting chemotherapy is given, then the cells are infused in a single sitting.
Some patients achieve deep and lasting remissions, but response depends on the disease type, the amount of cancer present, earlier treatments and your overall health. Cytokine release syndrome and neurological effects can occur in the first weeks and are managed by an experienced team, so it should be given only in a centre with that capability. Low blood counts and infection risk may continue for months.
Most programmes ask you to stay close to the treatment centre for about four weeks after the infusion, and to avoid driving during that time. A carer is usually needed. Ask about the manufacturing timeline, and arrange for your home haematologist to share blood tests and follow-up scans. Bring full records of earlier treatment.

- Enough time and stability to wait for cell manufacture, with bridging therapy if needed.

- Expect heart, lung, kidney and infection screening before cells are collected.
- Ask your doctor which medicines, especially steroids or other immune drugs, to adjust before collection.
- Plan to stay close to the hospital for several weeks after infusion; many centres require this.
- Arrange a caregiver who can stay with you, as you should not drive for a period.
