Vasospastic Angina

Vasospastic angina, also called Prinzmetal angina, is chest pain caused by a temporary spasm of a coronary artery. It often occurs at rest or at night and tends to affect younger adults than ordinary angina, particularly smokers.

vasospastic angina hakkında

Bilinmesi gerekenler

Pain is typically severe, comes in clusters, often between midnight and early morning, and may be relieved quickly with nitroglycerin. Diagnosis uses an electrocardiogram during an episode, ambulatory monitoring, and angiography with provocation testing. Spasm can sometimes cause dangerous rhythm problems, though with treatment the long-term outlook is generally good.

Treatment is medical. Calcium channel blockers are the first-line medicines, often with long-acting nitrates, and stopping smoking is essential. Beta-blockers are usually avoided. Stents or surgery are rarely needed unless fixed coronary narrowing is also present. Treatment abroad is rarely necessary because medicines are widely available.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • ECG recordings taken during an episode of pain, especially any showing temporary ST-segment elevation.
  • Holter or ambulatory ECG monitoring reports.
  • Coronary angiography results and images, which show whether there is fixed narrowing in addition to spasm.
  • Reports of any spasm provocation testing, which should only be done in a specialist setting.
  • A list of medicines, including calcium channel blockers and nitrates, and your history of smoking, cocaine or stimulant use.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You are advised to have a stent but angiography shows no fixed blockage.
  • Chest pain continues despite maximum medicines and the diagnosis is not clear.
  • You have had a dangerous rhythm, collapse or cardiac arrest with a diagnosis of spasm.
  • You were told your pain is not cardiac although it comes at rest, at night and responds to nitrates.

Doktorunuzla konuşurken

Sorulacak sorular

  1. How confident are you that spasm, rather than a fixed blockage, is causing my pain?
  2. Do I have any fixed narrowing that would justify a stent?
  3. Which medicines will control the spasm and how long will I need them?
  4. What triggers should I avoid, such as smoking or stimulants?
  5. What should I do if an episode of chest pain does not settle?
  6. Am I at risk of rhythm problems, and do I need monitoring or a device?

Prognoz

Gerçekçi prognoz

The outlook is usually good once the diagnosis is made and treatment is started. Calcium channel blockers, with or without long-acting nitrates, control symptoms in most people, and stopping smoking helps considerably. Dangerous rhythm disturbance or heart attack can occur but is uncommon with treatment. Stenting does not treat spasm and is only used when there is also significant fixed disease. Some people need long-term medicine, while others can have it reviewed.

  2. Önce yerel bir kalp cerrahisi uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Do I need a stent?

Usually not. Spasm is treated with medicines. A stent is considered only if angiography shows a fixed blockage as well.

Can it cause a heart attack?

Rarely, severe or prolonged spasm can cause heart attack or dangerous rhythms. Treatment and trigger avoidance lower the risk.

What triggers an episode?

Smoking, cold, stress, stimulants such as cocaine, and some medicines can trigger spasm. Episodes often occur at night or early morning.

Can I fly with vasospastic angina?

If your symptoms are controlled on treatment, usually yes. Carry your medicines and discuss travel with your cardiologist.

Clinical context

Working up vasospastic angina properly

A condition like vasospastic angina is rarely well-served by jumping straight to treatment shopping. The sequence below is what a kalp cerrahisi specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or kalp cerrahisi specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
