Uterine Synechiae (Adhesions)

Uterine synechiae, also called Asherman syndrome, are bands of scar tissue inside the womb cavity. They usually follow surgery such as curettage, or infection, and affect women of reproductive age.

uterine synechiae (adhesions) hakkında

Bilinmesi gerekenler

Common signs are very light or absent periods, painful periods and difficulty conceiving or repeated miscarriage. Diagnosis is made with hysteroscopy, saline ultrasound or hysterosalpingography. Without treatment adhesions may persist, and mild ones may be less likely to reform after removal than severe ones.

There is no medicine that removes adhesions. Treatment is hysteroscopic adhesiolysis, in which the scar bands are cut under direct vision. Afterwards, doctors may place a balloon or gel barrier, prescribe oestrogen and arrange a repeat check. Severe cases may need more than one procedure, and some women seek a gynaecologist with experience abroad.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • Hysterosalpingogram, saline sonogram or hysteroscopy reports describing the extent and position of adhesions.
  • A pelvic ultrasound or MRI report showing the womb lining thickness.
  • Hormone tests such as FSH, LH, AMH and thyroid function, to separate scarring from hormonal causes of absent periods.
  • A detailed history of the event that likely caused the scarring, such as a D&C, miscarriage surgery, caesarean or infection.
  • Records of any previous adhesion surgery, hormone treatment and pregnancy attempts.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You have severe or repeated adhesions after earlier treatment and want an experienced hysteroscopic surgeon to review.
  • You have absent periods and are unsure whether the cause is scarring, cervical blockage or hormonal.
  • You have been told your womb cannot be repaired without a full assessment by hysteroscopy.
  • You are planning IVF and want to know if the womb cavity should be corrected first.

Doktorunuzla konuşurken

Sorulacak sorular

  1. How severe and extensive are my adhesions?
  2. Can they be treated in one operation, or might I need several?
  3. What will be done after surgery to prevent the scars from re-forming?
  4. What is my realistic chance of regular periods and of pregnancy afterwards?
  5. Will my pregnancy need closer monitoring, and are there risks to the placenta?
  6. How and when will you check that the cavity is healing well?

Prognoz

Gerçekçi prognoz

Mild and moderate adhesions can usually be divided successfully, and periods often return to normal. Adhesions can reform, so a second look hysteroscopy is common. Pregnancy rates are good after treatment of mild disease but lower with severe scarring or a thin, damaged womb lining. Pregnancies after treatment carry somewhat higher risks of placental problems. Outcome depends on the severity of scarring and the condition of the womb lining.

Can adhesions in the womb come back?

Yes. Scars can re-form after they are cut, particularly in severe cases. Doctors often use measures such as hormone therapy or a second-look hysteroscopy to check.

Will my periods return to normal?

In many women, yes. Periods often return after adhesions are removed, though results depend on how severe the scarring was and how healthy the lining is.

Can I get pregnant after treatment?

Many women conceive after treatment, with or without fertility treatment. The chance is better for mild or moderate adhesions.

How is it treated?

A hysteroscope is passed through the cervix and the scar bands are divided under direct vision. It is usually a day-case procedure under anaesthesia.

Clinical context

Working up uterine synechiae (adhesions) properly

A condition like uterine synechiae (adhesions) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilite ve tüp bebek specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilite ve tüp bebek specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
