Omuz Artriti / Manşet Artropatisi

Shoulder arthritis, including cuff tear arthropathy, is wear of the shoulder joint that causes deep pain, stiffness and weakness. It is most often osteoarthritis in older adults, or follows a long-standing rotator cuff tear.

omuz artriti / manşet artropatisi hakkında

Bilinmesi gerekenler

Pain is deep in the joint and worse at night, with grinding, loss of movement and difficulty lifting the arm or reaching behind the back. X-rays show loss of joint space and bone spurs, and in cuff arthropathy the humeral head rides up. CT or MRI are used to plan surgery by assessing bone and the rotator cuff.

Initial care is painkillers, activity modification, physiotherapy and corticosteroid injections. If pain and function remain poor, shoulder replacement is effective. An anatomic total shoulder replacement suits an intact rotator cuff, while a reverse replacement is used when the cuff is torn or the joint is badly worn. Recovery involves a sling and months of rehabilitation. Patients travel for experienced shoulder surgeons.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • Shoulder X-rays from several angles, showing joint space loss, bone spurs and humeral head position.
  • CT or MRI to assess the glenoid bone shape and the state of the rotator cuff muscles and tendons.
  • Records of treatments tried, such as physiotherapy, anti-inflammatories and steroid injections, and how long relief lasted.
  • Your medical history, especially diabetes, inflammatory arthritis and previous shoulder surgery or fracture.
  • Blood thinners and other medicines, and any history of shoulder infection.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You have been advised to have an anatomic replacement but your rotator cuff is torn.
  • You are young, and a replacement may wear out in your lifetime, so alternatives are worth discussing.
  • You have had a previous shoulder replacement that is painful or loose.
  • Your pain is mild and replacement has been recommended without trying other treatment.

Doktorunuzla konuşurken

Sorulacak sorular

  1. Is my rotator cuff intact, and does that decide between anatomic and reverse replacement?
  2. What non-surgical options remain for me?
  3. What movement and pain relief can I realistically expect?
  4. How long will the implant last, given my age and activity?
  5. What are the risks of infection, dislocation and loosening?
  6. What are the rehabilitation phase and driving restrictions?

Prognoz

Gerçekçi prognoz

Shoulder replacement reliably reduces pain and improves function for most people, and both anatomic and reverse designs commonly last ten years or more. Reverse replacement suits those with torn rotator cuffs. Movement improves but may not be fully normal, and younger or very active patients have a higher chance of needing revision later.

  2. Önce yerel bir ortopedi uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Is shoulder replacement a major operation?

Yes, it is major surgery with a short hospital stay and several months of rehabilitation, but most patients are satisfied with pain relief.

What is a reverse shoulder replacement?

A design that reverses the ball and socket so the deltoid muscle powers the arm. It suits patients with torn or non-functioning rotator cuffs.

How long does a shoulder replacement last?

Commonly ten to fifteen years or more, though younger and more active patients may need revision sooner.

Clinical context

Working up omuz artriti / manşet artropatisi properly

A condition like omuz artriti / manşet artropatisi is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ortopedi specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ortopedi specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
